Case of a 64 years-old man with long standing persistent atrial fibrillation. First ablation consisted in extensive substrate ablation guided by artificial intelligence (Volta).
Remarkably, the patient had recurred with organized atrial tachycardia instead of atrial fibrillation, which was easily reduced with one shot at the critical isthmus.

57 years old man, palpitations for many years. Holter monitoring showed high burden atrial tachycardia. Mapping of the tachycardia revealed focal atrial tachycardia arising from the crista terminalis.

Le 23 septembre dernier avait lieu la première soirée « simulation en électrophysiologie » encadrée par les rythmologues de l’Institut Cardiovasculaire Paris Sud (ICPS).
Cette session s’est déroulée à l'Hôpital privé Jacques Cartier.
Dans une ambiance conviviale, une dizaine de rythmologues en formation ont pu s'initier ou se perfectionner en cartographie 3D de l'OG à travers 3 ateliers pratiques et avec le mannequin de simulation DAVID (Biosense Webster) installé dans nos salles de KT, au plus près des conditions de la vraie vie.

Case of a 58 years-old woman with an atypical atrial flutter, many years after atrial septal defect surgical closure. Mapping of the tachycardia showed an area of slow conduction in the atriotomy scar, allowing electrical conduction and macro reentrant tachycardia.

Feasability and diagnostic accuracy of stress perfusion MRI in patients with pacemaker is little known.
In this Research Letter just published in JACC CV imaging, 224 patients with MR-conditional pacemakers were prospectively referred for stress MRI and followed with a median follow-up of 7 years.
Image quality was good (in 84,1% of LV segments at stress) and device integrity was preserved.
In multivariate analysis, the presence and extent of inducible ischemia and Late Gadolinium Enhancement were independant predictors of major adverse clinical events.

Case of a 19 years-old man with scar related atrial flutter in a surgical corrected grown up complex congenital cardiomyopathy. The ablation was difficult due to an unusual venous access, with no possible catheterization of the inferior vena cava.

Evolution of high degree atrioventricular block after TAVI remains poorly understood, and identification of patients who will need permanent pacing is still a challenge.

STIM-TAVI was a prospective multicentre observational study designed to identify factors predicting late high AV blocks after TAVI.
