Root canal treatment on a right first molar (tooth #1.6) with a deep decay and acute apical periodontitis. 4 canal systems were located and shaped with DIRECT X ENDO instruments. A deep margin elevation (DME) was performed at the First appointment, than we carried out the root canal treatment, completing it with CWC technique and PCS (Kerr). A complex direct composite restoration of vestibular, palatal, mesial and occlusal surfaces was done at the end.

Root canal treatment of tooth #3.7 with irreversible pulpitis. Pre endo restoration was done using composite, then the root canal treatment was carried out using DIRECT X ENDO instruments (Directendo) and SS k files. Obturation was done with WVC technique and PCS by Kerr. After the root canal treatment, the tooth was restored with a direct composite restoration.

This case in an endodontic retreatment of tooth #2.6(upper left 1st molar).
The tooth came to our office in a bad situation, already opened, without a provisional neither definitive restoration, with a separated instrument in the MV root and an apical lesion in the palatal one.
Thanks to the presence of the 4th canal we were able to shape and clean the mesial root, using SS k-files (06-08-10), than a rotary 14.04(X1) of the DIRECT X GOLD instrument (DIRECTENDO) and at least SS k-files 15 and 20. I didn't force the removal neither the bypass of the fractured instrument in the root, considering the mesial canals merging each other.
For DV and P canals we used DIRECT X GOLD instruments up to X4, refining the foramina up to .50 (palatal) and .45 (disto-vestibular).
Obturation was done with warm vertical compaction and PCS (KERR).

After a direct restoration, we completed the case with a direct composite overlay, about two months after the retreatment.
