Thanks to a strong European collaboration, after a long work, we have been able to identify the pathophysiological phenotypes of moderate - severe BPD in extremely preterm infants. Current BPD definitions do not capture these traits and do not identify these patients early enough to allow a treatment, let aside a personalized treatment. This is pivotal every baby is different and experimental therapies (such as r-hu-IGF1/IGFBP, r-hu-SPD and MSC-EVs) are under advanced investigations and look promising. Babies developing more severe BPD have peculiar lung ultrasound finding with loss of lung aeration and impaired oxygenation early in life. Looking at these pathophysiological phenotypes, a targeted treatment will be possible.
