Bucks Community Wellbeing Hub in 📍Aylesbury town centre have recieved funding from the Aylesbury Community Board to run a Creative Health and Wellbeing Programme this Autumn!

✨Patient Need Groups 👥 - SEGMENTATION✨

ℹ️ What are they and how are they used in healthcare?

🤔What are the John Hopkins Patient Need Groups (PNG)?

Patient Need Groups categorise patients into different groups based on the complexity and intensity of their healthcare needs rather than just their medical conditions.

Instead of focusing solely on a diagnosis (like Diabetes or Heart Disease) it takes in to account a broader range of health characteristics about a patient.

❗️You may notice a test result appear in your NHS APP referring to Segmentation. Segmentation is a simple way of categorising patients based on their specific health needs. It helps us as a practice understand the individual needs of our patients based on their health and wellbeing to support personalised care and ensures you get the right support in a timely way. You do not need to worry about this or contact the practice❗️

🤔 How Do PNGs Work?

These groups consider factors like the number of chronic conditions, the severity of illness and the level of support a patient might need.

The patients in a particular group have similar patterns of need.

For example:

🔴 Red PNG 10,11 🔴 = Complex Care Needs: Patients with multiple chronic conditions, requiring regular monitoring and a coordinated care approach.

🟠 Amber PNG 5-9 🟠 = Stable Chronic Conditions: Patients with chronic conditions that are well-managed and require occasional support to maintain their stability.

🟢 Green PNG 1-4 🟢 = Healthy: Patients without significant health issues who could benefit from preventive care.

As patient needs change over time, their categorisation changes with them.

PNG offers an at-a-glance indication of a patient’s broader needs, at a point in time.

🤔What are the benefits of having this information available on patient records?

1️⃣ Personalised Care: Healthcare providers can use PNGs to tailor their care plans. This means patients receive the specific type of care they need, whether it’s more frequent monitoring, specialised treatment, or preventive care.

2️⃣ Improved Coordination: PNGs help healthcare teams coordinate more effectively. If a patient has complex needs, the care team can work together to ensure all aspects of their health are addressed, from medication management to lifestyle support.

3️⃣ Better Resource Allocation: Hospitals and GP Surgeries can allocate resources more efficiently based on the needs of different patient groups. This ensures that patients with the most urgent needs receive timely care, while those with less critical conditions are managed appropriately.

4️⃣ Proactive Health Management: For patients in lower-need groups, PNGs encourage preventive care and early intervention. This can help them maintain their health status and avoid complications in the future.

5️⃣ Enhanced Communication: PNGs facilitate better communication between patients and healthcare providers. By understanding which group a patient falls into, together they can have clearer discussions about what to expect from the care they receive and what actions they can take to manage their health.

Who is eligible for Mounjaro from GPs right now?

The NHS has recently released the criteria for accessing weight loss injections via your GP Surgery.

The first phased cohort eligible for Mounjaro on the NHS is only for those with BMI above 40 and with 4 of 5 defined comorbidities:-
1. Diagnosed hypertension
2.Diagnosed sleep apnoea
3.Diagnosed type 2 diabetes
4.Diagnosed cardiovascular disease
5.Diagnosed hypercholesterolemia

If you do not have a BMI above 40 and four of the conditions above, Poplar Grove will not be able to prescribe you Mounjaro for weight loss. We have received many queries from patients wishing to see a GP to discuss their eligibility for this new service, resulting in unnecessary appointments for patients that do not qualify for Mounjaro injections under the NHS.

We have identified the patients eligible for this NHS service, and we will be in contact with them directly to discuss this further.

PRESCRIPTION PHONE LINE UPDATE

Following on from patient feedback, our dedicated Prescription Queries Phone line will now be open from the new time of 8am - 10am, Monday to Friday.
Remember- the easiest way to order repeat prescriptions is:
•Through in the NHS App
or
•Complete an order form by hand and post into the prescription box in our waiting room or outside mailbox.

We do not take repeat prescription requests over the phone or email.

We kindly ask all Patients follow these guidelines when requesting medication:
•Order your repeat prescription at least 1 week before you are due to run out.

•Order earlier than 1 week if there are bank holidays coming up.

•Please allow us 5 working days to process prescription request.

Many calls to our Prescription line are to chase if a Prescription is ready for collection. We encourage patients to track the progress of prescription on the NHS App to avoid unnecessary calls being made to the GP Practice and the Community Pharmacists. Please do not call us unless 5 days have passed since you requested your medication.
