We work in partnership with the NHS continuing healthcare teams, local authorities to provide our enhanced level of specialist clinical support in the community.
As a Homecare Lead Nurse, you would be responsible for mentoring carers, providing clinical support to the care manager and improving lives of service user’s in the community. Some of the conditions you may come across are Epilepsy, Brain injury, spinal cord injury, Tracheostomy/ventilation, nutrition needs and learning disability etc.
Working hours could be part time flexible hours.
Your role will involve in executing the care prescription provided. However, equipment and various other treatments are provided and maintained by the authorities.
Key Requirements
• Registered Nurse with a minimum of 12 months’ work experience in the UK and an active NMC PIN
• Ability to build strong relationships with clients, their families and carers
• Confident, passionate and possess a “can-do” attitude
• Supportive management style that can bring out the best skills of carers.
• Excellent written & verbal communication skills
• Ability to take ownership of your role and work flexibly to meet requirements of service users.
• Desirable but not essential experience; Tracheostomy, Home ventilator experience, Care of the spinal patient, PEG/PEJ feeding, Catheter care.( Required training and support will be provided free)
Working set up
a) Client’s locations are within 25 miles radius from Aylesbury, currently in Aylesbury and High Wycombe.
b) Except for checking and completing paperwork completed by carers you are not required to regularly be in the office we could work around your availability provided you are able to be available within 3 to 5 days’ notice.
c) Tasks completed would be recorded in a rostering app which would be used to make payments
What you would do routinely:
a) MAR chart checks before posting against any medication changers as informed by GP (Once a month)
b) Review & sign paperwork; daily care notes and monitoring forms and MAR charts.
c) Ensure service user folders are accurate and up to date reviewing when care need changers.
d) Epilepsy, medication administration and peg training (other training will be completed by training director)
e) Attend initial assessments of new SU to verify requested care needs when needed
f) Support carers with clinical skills and help maintain competency
g) Review care plans & Risk assessments when needed
h) Prepare and attend multi-disciplinary meetings if needed
