After supporting nurses to OSCE success since 2019 as a clinical educator, I’ve had to adjust through the changes in the exam, as well as candidates. It was a massive upheaval - a constantly moving target, and I was updating my programme pretty much every day for a while. The changes in the exam are now nicely embedded and less edits to the marking criteria etc. ⁣⁣

I now enjoy teaching and supporting nurses for the updated Test of Competence (Part 2-OSCE) far more than I did for Legacy OSCE (6 station-older format). ⁣⁣

Why? Rationale and evidence-base. ⁣⁣It's less contradictory now.
⁣⁣
Because it’s now aligning more closely with everyday clinical practice and everything is linked. Instead of me saying “𝘋𝘰 𝘵𝘩𝘪𝘴 𝘵𝘩𝘪𝘯𝘨 𝘵𝘰 𝘱𝘢𝘴𝘴 𝘖𝘚𝘊𝘌 𝘣𝘶𝘵 𝘯𝘰𝘵 𝘢𝘤𝘵𝘶𝘢𝘭𝘭𝘺 𝘪𝘯 𝘤𝘭𝘪𝘯𝘪𝘤𝘢𝘭 𝘱𝘳𝘢𝘤𝘵𝘪𝘤𝘦” (Legacy-old style OSCE), I’m now frequently saying “𝘛𝘩𝘪𝘴 𝘪𝘴 𝘶𝘱 𝘵𝘰 𝘥𝘢𝘵𝘦 𝘨𝘶𝘪𝘥𝘢𝘯𝘤𝘦, 𝘸𝘪𝘭𝘭 𝘩𝘦𝘭𝘱 𝘺𝘰𝘶 𝘱𝘢𝘴𝘴 𝘖𝘚𝘊𝘌, 𝘴𝘶𝘱𝘱𝘰𝘳𝘵 𝘱𝘢𝘵𝘪𝘦𝘯𝘵 𝘴𝘢𝘧𝘦𝘵𝘺 𝘢𝘯𝘥 𝘮𝘢𝘺 𝘦𝘷𝘦𝘯 𝘩𝘦𝘭𝘱 𝘺𝘰𝘶 𝘢𝘯𝘴𝘸𝘦𝘳 𝘢𝘯 𝘪𝘯𝘵𝘦𝘳𝘷𝘪𝘦𝘸 𝘲𝘶𝘦𝘴𝘵𝘪𝘰𝘯” – This is GREAT news! ⁣⁣

𝐎𝐒𝐂𝐄 𝐩𝐫𝐞𝐩𝐚𝐫𝐚𝐭𝐢𝐨𝐧 – 𝐠𝐞𝐭 𝐲𝐨𝐮𝐫 𝐚𝐜𝐭𝐢𝐧𝐠 𝐬𝐤𝐢𝐥𝐥𝐬 𝐨𝐧 𝐩𝐨𝐢𝐧𝐭! As the educator, I frequently take on the role of patient actor while I am assessing. Often, I am the nurse demonstrating. Sometimes, I am all 3 at once, which is fun! 🤒🤕😆👀📝

Like me, our nurses get fully involved and learn how to verbalise what they are doing and why.

They take turns being the patient actor-yes, even online, and they also get to be the assessor.

𝘛𝘩𝘪𝘴 𝘪𝘴 𝘢 𝘬𝘦𝘺 𝘱𝘢𝘳𝘵 𝘰𝘧 𝘖𝘚𝘊𝘌 𝘱𝘳𝘦𝘱𝘢𝘳𝘢𝘵𝘪𝘰𝘯 𝘢𝘯𝘥 𝘦𝘹𝘵𝘳𝘦𝘮𝘦𝘭𝘺 𝘨𝘰𝘰𝘥 𝘧𝘰𝘳 𝘣𝘶𝘪𝘭𝘥𝘪𝘯𝘨 𝘤𝘰𝘯𝘧𝘪𝘥𝘦𝘯𝘤𝘦 𝘢𝘯𝘥 𝘤𝘰𝘮𝘱𝘦𝘵𝘦𝘯𝘤𝘦.

𝐖𝐡𝐲? This allows nurses, under simulated circumstances, to see and understand things from different perspectives, picking up on subtle cues and nuances that they may miss otherwise.

𝐃𝐢𝐯𝐞𝐫𝐬𝐢𝐭𝐲 𝐚𝐧𝐝 𝐢𝐧𝐜𝐥𝐮𝐬𝐢𝐨𝐧. Like our patients, we are all from different backgrounds. Everyone’s perspective is relevant and acknowledged.

Peer feedback should always be constructive, professional and has the potential to invoke discussion, deep understanding of UK nursing guidelines and the power of clear communication.

𝐓𝐡𝐞 𝐩𝐮𝐫𝐩𝐨𝐬𝐞 𝐨𝐟 𝐚𝐥𝐥 𝐨𝐟 𝐭𝐡𝐢𝐬 '𝐚𝐜𝐭𝐢𝐧𝐠' ?

Patient and observer perspective, as well as the nurse’s, helps us to understand and really ‘get into’ each scenario.

𝘛𝘩𝘪𝘯𝘬 𝘰𝘧 𝘦𝘢𝘤𝘩 𝘴𝘤𝘦𝘯𝘢𝘳𝘪𝘰 𝘯𝘰𝘵 𝘫𝘶𝘴𝘵 𝘢𝘴 𝘸𝘰𝘳𝘥𝘴 𝘰𝘯 𝘢 𝘱𝘢𝘨𝘦 𝘣𝘶𝘵 𝘢 𝘱𝘦𝘳𝘴𝘰𝘯’𝘴 𝘴𝘵𝘰𝘳𝘺 𝘢𝘯𝘥 𝘦𝘹𝘱𝘦𝘳𝘪𝘦𝘯𝘤𝘦.
Really dig into it - what setting am I working in? Who is the patient? What do they like to be called? What are their concerns? What is their history?

𝐑𝐞𝐚𝐝, 𝐥𝐢𝐬𝐭𝐞𝐧 𝐚𝐧𝐝 𝐫𝐞𝐬𝐩𝐨𝐧𝐝 𝐚𝐜𝐜𝐨𝐫𝐝𝐢𝐧𝐠𝐥𝐲.

As the nurse, how would you like your words and actions to be remembered in that patient's story?

How would you feel if you were the patient in that moment? Or as the assessor and as the nurse.

Did it make you feel comfortable to witness? Anything you would change?

As the patient, did the nurse ask you for consent by giving you relevant information first?
Or did they just go ahead and start without asking you if it was okay with you first?
Did they listen to your replies and respond accordingly? Or did they speak over you, ignoring your reply and asking you the next question.

How was their manner and communication?
Clear and compassionate or stilted and lacking in clarity.

So, you see - OSCE preparation is not about checklists. It’s about people and patients as the priority.

We must truly understand rationale for each intervention, treating each person as an individual, rather than learning the marking criteria and saying the ‘right’ words.

Dont forget 💡You are also still allowed to be yourself throughout the process.

The best part of my job? Getting to support other nurses with their goals, on their own timeline 💜

How does our flexible approach to OSCE preparation and support actually work in practice? Why do our learners achieve high first-time pass rates, retain their learning for post OSCE clinical practice and recommend us to others?

This week, we have:

• Continued supporting a small group of nurses who have been preparing for months and are sitting OSCE soon at Ulster CTC (NMC OSCE Centre).

• One nurse has just arrived in the UK for final face to face sessions to consolidate their learning, which has been ongoing online, for a number of months.

• The other nurse is already living in the UK and has been working in a different health and social care role for the last 2 years. This nurse has also been preparing for months and has been attending face to face sessions in the last 2 weeks.

Both are making excellent progress!

We have covered all theory, demonstrations and strategies online, with both nurses attending all online sessions and fully commited to their self-directed learning.

This hard work is demonstrated in each session, with their confidence growing each day.

These practical sessions consolidate all previous learning and allow our nurses to continue building confidence and competence for the big day & beyond.

We are delighted to recently welcome 3 nurses to our supportive OSCE programme. They had access to their e-learning from day 1 and their first sessions online this week.

3 wonderful nurses from a variety of circumstances, already committed and working hard to achieve OSCE success.

Looking forward to meeting them in person when they arrive in the UK in the coming months.

Programme learner success rate- 100%.

Since 2019, we have supported over 450 nurses to OSCE success and have worked with health care professionals and nurses pre, during and post OSCE preparation. ⁣
We have experience in a variety of senior, quality assurance, education and 'bedside' UK nursing roles and truly understand the current challenges for both internationally and UK educated nurses. ⁣
