Mae Cynghorau Iechyd Cymuned yn helpu pobl i dderbyn y gwasanaethau angenrheidiol iddynt hwy a'r rhai maent yn gofalu am. •Ceisiwn farn cleifion a'r am wasanaethau lleol a defnyddiwn y wybodaeth honno i lywio ein gwaith gyda'r GIG Ni yw'r ddolen rhwng y rhai hynny sy'n cynllunio a darparu'r gwasanaeth a'r rhai hynny sy'n defnyddio'r gwasanaeth. •Gwrandwn ar yr hyn sydd gan unigolion a'r gymuned i'w ddweud am eu gwasanaethau iechyd lleol ag ystyried ansawdd, yr hyn sydd ar gael a hygyrchedd. Wedyn, byddant yn lleisio barn y cyhoedd drwy adael i reolwyr gwasanaethau iechyd wybod pa welliannau yr hoffai pobl weld. •Rhaid i ddarparwyr gwasanaethau iechyd lleol ymgynghori a ni ynglyn ag unrhyw newidiadau i wasanaethau iechyd o fewn eu hardal. Ymgynghorwn gyda'r cyhoedd i sicrhau eu bod yn trosglwyddo gwir farn y cyhoedd i ddarparwyr iechyd lleol neu i Lywodraeth Cynulliad Cymru. •Byddwn yn arolygu safleoedd y GIG ac yn gwneud argymhellion ar gyfer gwelliant lle bo angen •Byddwn yn craffu ar y GIG ac yn cydweithio gyda chynllunwyr a darparwyr gwasanaeth i wella profiad y claf o'r gwasanaethau. •Yn ogystal, rydym yn helpu i gynghori a chefnogi pobl sy'n dymuno gwneud cwyn am wasanaethau GIG. CHCs help people get the health services they need for themselves and those that they care for. •we seek the views of patients and the public on local services and use that information to inform our work with the NHS. We are the link between those that plan and deliver the service and those that use the service. •we listen to what individuals and the community have to say about their local health services with regards to quality, availability and access. We then act as the public voice in letting managers of health services know what people want and the improvements they would like to see. •we must be consulted by the providers of local health services about any changes to health services within our area. We must then in turn talk to the public to make sure they are properly reflecting the views of the public to the local health service providers of the Welsh Assembly Government •we inspect NHS premises and make recommendation for improvement where necessary •we scrutinise the NHS and work with service planners and providers to improve the patients experience of services •we help, advise and support people who wish to make a complaint about NHS Services.
