Breastfeeding & Teething🦷👶

​It’s the moment every breastfeeding parent dreads: that first little pearly white sharp tooth breaks through the gums 🦫

​Your first thought is probably, "Is this the end of my pain-free latch?"
​Take a deep breath. You can absolutely continue your breastfeeding journey comfortably through the teething stage.

Here is a clinical secret: A baby physically cannot bite you while they are actively sucking and swallowing, because their tongue naturally covers their lower teeth to create a latch!
​Biting usually happens at the very beginning or the very end of a feed when they are distracted, full, or trying to massage their sore gums.

​Leah’s Midwife Tips for Teething Babies:
​🧊 Cool Down First: Before a feed, give your baby something cold to chew on—a chilled teething toy, a clean wet flannel, or a piece of cold food if they are on solids (cucumber works well 🥒). This numbs their gums so they don't use you for friction.
🧐 Watch for the Shift: Pay close attention to the end of the feed. When their jaw relaxes, they stop actively swallowing, or they get a mischievous look in their eye—slip a clean finger into the corner of their mouth to break the suction before they can clamp down.
🚫 The "No Drama" Reset: If they do nip you, try not to scream (easier said than done 😬!). A loud reaction can sometimes startle them into doing it again, or make them think it's a fun game. Instead, say a calm, firm "No," break the latch, and put them down for a minute. They quickly learn: Biting means the milk goes away.

Do you know how to recognise those early feeding cues? 👀

​In those early weeks, it can feel like a guessing game, but your baby has a very specific set of feeding cues. If we can spot them early, feeding is usually much calmer for both of you.

​Think of it like a Traffic Light system:
​🟢 Early Cues (I’m getting hungry):
​Licking lips or sticking tongue out.
​Smacking lips or making sucking sounds.
​Opening and closing their mouth
​🟡 Mid Cues (I’m really hungry now!):
​Rooting (turning their head searching for the breast/bottle).
​Trying to suck on their hands or fingers.
​Fidgeting, squirming, or "cycling" their arms and legs.
​🔴 Late Cues (I’m upset and overwhelmed):
​Crying and screaming.
​Turning red in the face.
​Frantic, jerky movements.

​Leah’s Midwife Tip:
If your baby has reached the "Red" stage, they might be too upset to latch properly. Try a little skin-to-skin or some soothing cuddles to calm them down before you try to feed.

It’s much easier to learn to feed a "Green" baby than a "Red" one!

Did you know your breastmilk changes when you or your baby is sick? 🤒🤮

​It sounds like magic, but it’s pure biology. Your body is constantly scanning your baby’s health every time they latch.

🦠​How it works:
When your baby is sick, their saliva back-flows into your ni**le (the "vacuum" effect). Your body detects the pathogens—the germs or viruses—in that saliva.

​What happens next is incredible:
🌡️ The Immune Boost: Within hours, your body creates specific antibodies tailored exactly to whatever bug your baby is fighting.
🧪 The Color Shift: You might notice your milk looks more yellow, orange, green or even blue during this time—that’s often due to a massive surge in white blood cells (leukocytes) to help fight the infection.
💧 The Hydration effect: If your baby has a high temperature, your milk naturally becomes higher in water content to keep them hydrated.

😷​And if YOU are sick?

👣 Fresh Air Therapy: Even if it’s just standing on the doorstep for 2 minutes of deep breathing, the change of scenery helps.

💛"Soft" breasts do NOT mean you’re running out of milk.💛


💫​If you’ve reached the 6–12 week mark and suddenly noticed your breasts feel soft, "empty," or no longer leak—congratulations! Your body has actually just become an expert at its job.

🩷​Here is the clinical truth:
In the early weeks, your body is hormonal and over-produces milk because it doesn't know how much your baby needs yet. This leads to that "engorged" full feeling.

​Eventually, your body moves from hormonal regulation to supply and demand. It starts making milk in "real-time" as your baby asks for it.🩷

​Think of your breasts as a tap, not a jug.
❌ A jug can be empty.
✅ A tap is always ready to flow as soon as it's turned on.

​If your breasts feel soft but:
✨ Your baby is still having 6+ heavy wet nappies a day.
✨ Your baby is settled (mostly!) after a feed.
✨ Your baby is meeting their weight milestones.

💚 ​Here’s the clinical truth: A baby doesn't just "suck" on a bottle; they use their tongue to create a vacuum and seal. If the tongue is restricted, that seal is broken, and feeding becomes a battle of physics.

During our home visit, we don't just look at the tongue. We look at the bottle, the teat flow, and your positioning.
Sometimes "Paced Bottle Feeding" is the answer; sometimes a division is needed to give that tongue the freedom to work. Every baby is different.

The Truth About Lip Ties & why I dont cut them 👄✨

​It is one of the most common questions I get during a home visit: "Leah, my baby has a lip tie too, can you cut that while you're here?"
​My answer is always no. As a Specialist Tongue Tie Midwife and IBCLC, I follow the evidence—and currently, in the UK, there is no robust evidence to show that "releasing" a lip tie improves feeding.
​Here is why we leave them alone:
​🦷 Dental Development: Almost all babies have a "labial frenulum" (the tissue connecting the lip to the gum). As the jaw grows and teeth come in, this tissue usually moves higher up, and the gap naturally closes.
🤱 Feeding is about the Tongue: The "engine" of feeding is the tongue. If the tongue can lift and move correctly, a baby can still get a great seal, even if their top lip doesn't "fl**ge" perfectly like a textbook.
⚠️ The Risks Outweigh the Benefits: Cutting a lip tie involves a higher risk of bleeding and, more importantly, a high risk of the tissue re-attaching or scarring, which can cause more problems later on.

​What I focus on instead:
I look at the whole picture. We work on the tongue’s function and the "asymmetrical latch." Most of the time, once the tongue is moving well, the tension has been improved or the positioning is tweaked, the "lip tie" becomes a non-issue!

In the rare event that your baby's lip tie is significant and causing major issues we can work on exercises to improve mobility and I can refer you to the relevant health professionals to get this addressed.

👀Common misconception: "My baby has a tongue-tie, so they need it cutting."👀

In my practice: We treat the function, not just the look.

Many babies have a visible frenulum (the bit of skin under the tongue), but they can still lift, extend, and move their tongue perfectly to feed without pain. If your baby is gaining weight well and feeding is comfortable and enjoyable, we leave it be if thats what you want!

When do we consider a division (Frenulotomy)?
We look for the "symptoms" of restricted movement, such as:
🚩 Difficulty staying latched (slipping off).
🚩 Clicking sounds during feeds.
🚩 Significant pain or damage for the breastfeeding parent.
🚩 Excessive gas or reflux symptoms.
🚩 Poor weight gain.
🚩 Concerns around milk supply.

My Approach:
During our home visit, I perform a full clinical assessment. Often, we can fix the issues described above with simple positioning and attachment tweaks —no procedure required!
My goal is always to find the gentlest path to a happy feeding journey for you and your baby to meet your goals.
