Social prescribing Link Workers are present in surgeries across the Wirral to help patients identify and link with sources of support within the community. These non-medical referral options can complement existing medical treatments to improve health and wellbeing.
Social Prescribing Link Workers are not medical professionals but have been specially trained to work alongside GPs and other clinical roles to ensure people get the care and support they need.

The role of a Social Prescribing Link Worker is to give people a chance to talk, listen to them carefully, offer emotional and practical support and provide them with the advice, information and support they need whatever their worries, questions or concerns.
If you would like to speak to a Social Prescriber, please enquire at your next appointment, via PATCHS or by calling into the practice and giving your details to our Medical Receptionist.

The Living Well Bus supports our communities to live well through:
• COVID-19 vaccination
• Blood pressure
• Pulse checks
• Cholesterol check
• Blood glucose check
• BMI check
• Direct referrals to talking therapies. (Mental Health)

Upcoming Wirral visits from the Living Well Bus, operated by Cheshire and Wirral Partnership NHS Foundation Trust are as follows:

Monday 3 April
• 10.30am - 1pm at Victoria Park, Bebington Road Entrance, Rock Ferry, CH42 6SH
• 1.30 - 4pm at Woodhead Street Car Park, Woodhead Street, New Ferry, CH62 5ER
Saturday 15 April
• 10.30am - 4pm in Birkenhead town centre, between Primark and ASDA
