Another great 6 month review of a large lesion healing in a lateral incisor.
These cases are always consented to a possible surgical option but first and foremost the tooth needs a root canal treatment.
If the tooth can be thoroughly cleaned, shaped and filled. Then in many cases despite the size of the lesion healing can occur.

The case was treated over 2 visits, with Ca(OH)2 placed to the full length of the canal.
Obturation was with OneFill and GP and at 6
Month review bony infill was seen on the review Radiograph. 😃

3 month recall with excellent healing.
Patient referred for re treatment after a large abscess was detected.
I suspected this case would require a surgical retreat as the lesion was so large, but at 3 month post op the pathology had nearly completely resolved.

Laterals, very common in the apical third! Once I’ve shaped the main canal I will go back in with a size 10 k-flexofile which is precurved in last mm. Feeling around the walls of the canal I Frequently find another portal of exit which could harbour bacteria and pulp remnants which could lead to failure of the case. Activated irrigants and hydraulic obturation techniques complete the job!

Treating endodontic disease whilst leaving the smallest footprint possible.

Modern files and irrigation techniques allow for cleansing of the apical third without loosing the pericervical dentine.

Couple of curvy molars.
Maximum preservation of the pericervical dentine whilst fully shaping the complex anatomy.
Diagnosis: pulpal necrosis with dental abscess.
UL7 treated first canals shaped with VS flexi blue to a master file 25/06
Obturated with one fil and GP
UL6 also shaped with VS Flexi Blue to a 25/06 and obturates with One fill BC sealer and GP.
Both teeth had mb2s merge with the MB1 at the curvature.
Some excess sealer from the 7 but nothing I’m overly concerned about.
