This new study examined whether interoception and multisensory integration predict physical functioning in older versus younger adults. Within the older adult group (age 58-87 years), interoceptive sensibility, interoceptive awareness, and multisensory temporal resolution were significant predictors of physical functioning. Better interoceptive sensibility and awareness were associated with enhanced functioning, whereas poorer temporal resolution corresponded with reduced physical abilities.

โ–ช๏ธ ๐ŸŒ Musculoskeletal (MSK) painโ€”encompassing conditions like back, neck, shoulder, and knee painโ€”is the leading cause of disability globally, yet the quality of healthcare provided for these conditions is often suboptimal.
A systematic review by Lin et al. highlights a significant gap between evidence and practice, characterized by the overuse of radiological imaging, surgery, and opioids, alongside a failure to provide patients with essential education and advice.

โ–ช๏ธ ๐Ÿ“˜ To address this, researchers analyzed 44 clinical practice guidelines (CPGs) for spinal, hip, knee, and shoulder pain, identifying only 11 that met high-quality standards.
From these high-quality guidelines, they distilled 11 consistent recommendations that constitute best practice care for MSK pain, regardless of the specific body part affected.

It is essential to assess factors such as depression, anxiety, fear of movement, and recovery expectations, as these are critical prognostic indicators.

A physical exam including mobility, strength, and neurological screening is recommended to assist in diagnosis and classification.

Radiological imaging (X-rays, MRIs) should not be used routinely. It is discouraged unless serious pathology is suspected, there has been an unsatisfactory response to conservative care, or the results are likely to change management strategies.

This is a cornerstone of management; guidelines consistently recommend physical activity, aerobic exercise, or specific strengthening exercises depending on the condition.

โ–ช๏ธ Manual Therapy as an Adjunct:
Manual therapy (hands-on treatment) should only be applied in conjunction with other evidence-based treatments, such as exercise and education, rather than as a standalone therapy.

โ–ช๏ธ Non-Surgical Care First:
Unless there is a specific "red flag" indication, patients should be offered evidence-informed non-surgical care prior to any consideration of surgery.

โ–ช๏ธ Facilitate Return to Work:
Clinicians should actively facilitate the continuation or resumption of work, as staying active is generally beneficial.

โ–ช๏ธ Monitor Progress:
Patient progress should be evaluated using validated outcome measures to ensure the chosen management strategy is working.

Arthroscopic lavage and debridement (clean-out surgery) should not be performed for knee OA unless there is a specific rationale like mechanical locking. Additionally, glucosamine and chondroitin are not recommended for disease modification.

Paracetamol should not be offered as a single medication, and opioids should not be offered for chronic low back pain. Furthermore, spinal injections and disc replacement are generally discouraged.

For all therapists (physios, OTโ€™s, osteopaths etc) who are interested in Human Movement.

Working with patients to help them to understand and explore their movement boundaries. Promoting more positive movement patterns, reducing discomfort and improving confidence

There is a slight skew towards neurological disorders and elderly rehabilitation, but it is relevant to all therapists who have patients whose movements are limited through:
โ€ข ingrained poor patterns of movement, with reduced awareness of their body
โ€ข fear of movement due to chronic pain or falls
โ€ข changes to neurological systems (stroke/MS/PD FND etc),

The focus is on human movement and how to improve confidence, through safe and controlled exploration of boundaries.
Helping your patients to:
โ€ข Understand their own movement and how/why it is limited
โ€ข promote self exploration
โ€ข expand their movement boundaries
โ€ข experience positive movement again,

The day will include an overview of:
โ€ข how altered sensory feedback (eyes, vestibular app, sensation, proprioception) effects โ€˜normalโ€™ human movement
โ€ข How we can direct our patientsโ€™ attention to alter sensory feedback, changing how that sensation is perceived
โ€ข How state of mind (fear, expectations etc) alters human movement
There will be a large practical element where we will explore:
โ€ข Building rapport/trust and leading/following techniques
โ€ข Simple use of breath to alter state.
โ€ข Self-exploration of own body and altered movement patterns/sensory feedback to gain personal experience of how change happens.
โ€ข How to quickly build โ€˜groundingโ€™ and controlled, safe self-exploration of boundaries to build better movement.
โ€ข Patient case studies and discussion of how you can use the treatment techniques with a wide variety of your patients.

Alyson Smith is a specialist neuro physiotherapist with over 30 yearsโ€™ experience in rehabilitation.
She has developed techniques which address the person, learning how to build good rapport with individuals (and their central nervous system) and has a thorough understanding of how a person's sensory feedback and state of mind has a profound effect on the way they move.
Alyson has delivered training to NHS Trust Therapy Departments, Essex University physiotherapy course, Community Rehabilitation Teams, Neurology Rehabilitation Teams, Headway, PDA, Osteopathy conferences and Nursing Homes around Essex.
This course is relevant to all those therapists who treat people with diminished mobility.
