Consultant-led laser treatment for Rhinophyma at Midland Skin

This fantastic patient result shows exactly how ablative CO₂ / Erbium laser can reduce the thickened, lumpy nasal tissue of rhinophyma, restoring a more natural contour and smoother skin texture. For this patient, the procedure also improved quality of life significantly; as their rhinophyma was blocking the airways and impacting breathing

Rhinophyma treatment at Midland Skin is precision procedure performed under local anaesthetic, designed to remove excess tissue with reduced bleeding, shorter downtime (healing ~10 days; most back to normal by ~2 weeks) and minimal scarring, with durable results. One or two sessions are typically planned, with a second stage for fine balance where needed.

For earlier rhinophyma, we also offer lighter/fractional ablative laser and a 1726 nm sebaceous-gland–specific laser to help limit progression and support long-term control.

Can AviClear help rosacea? 💭
Short answer: sometimes - in the right patient. As you can clearly see in the before and after results above ⬆️

While AviClear® was developed for acne, its ability to dial down sebaceous gland activity can be useful for certain rosacea subtypes — particularly papulopustular rosacea (inflamed spots/pustules). In carefully selected patients we also see softening of early nasal swelling (early phymatous change/rhinophyma), because reducing oil gland overactivity can calm that tissue response.

What it’s not for: persistent flushing, background redness or visible capillaries - those usually need vascular lasers, topicals or prescription care. The key is precision diagnosis and a tailored plan.

Dr Rajpar’s approach:

✔️ Diagnose the rosacea subtype(s) accurately
✔️ Consider AviClear where oil-gland suppression will help inflammatory lesions or early phymatous change

Before & after: Venous lake treatment at Midland Skin

Venous lakes are small, dark vascular spots that often appear on the lips. Using the 1064 nm Nd:YAG laser (Cutera Excel V+), they can be treated quickly and effectively ⚡️

During treatment the spot darkens or whitens and becomes firmer – signs the vessel has sealed. Lasers without deep enough pe*******on won’t fully clear the lesion… but this before and after results papped immediately post-procedure, which shows fantastic clearance, instantly!

Rhinophyma treatment is one of the most rewarding procedures in dermatology. Many patients have lived with the condition for years, often feeling stigmatised, before seeking help. Treatment is highly effective, with excellent long-term results.

Several ablative lasers can be used, and at our clinic we have access to them all. Dr Rajpar’s preferred device is the UltraPulse CO₂ laser. The CO₂ wavelength provides immediate haemostasis, allowing the procedure to be carried out more efficiently.

The laser must be able to deliver a sufficient power to vaporise excess tissue while minimising residual thermal damage. Some thermal effect is inevitable and must be anticipated during contouring, which is where the true skill and artistry of treatment lies!

Have you heard of venous lakes before? 💭

Before & after: venous lake treatment at Midland Skin

What is a  ? 👇

A venous lake is a benign vascular lesion that typically appears as a small, dark blue to purple papule on the skin. They’re caused by dilated venules (small veins) and are most commonly found on sun-exposed areas… particularly areas like the lips, face, ears and neck.

What causes them?

There are a few causes and risk factors to note when it comes to venous lakes… my number one tip in preventing them being always using a broad spectrum SPF, seeking shade when the sun is at its strongest & investing in a wide brimmed hat; preventing the development of venous lakes by reducing sun damage to the skin

▪️ Sun exposure can cause venous lakes - chronic sun exposure increases your risk of developing them
▪️ Ageing is also a factor - these skin lesions are more common in older adults, likely due to cumulative sun damage and changes in skin elasticity
▪️ Genetics - some people may also have a genetic predisposition to developing venous lakes

How can we treat them?

Venous lakes are benign and asymptomatic, meaning that treatment is often not necessary unless they cause you cosmetic concerns or recurrent bleeding.

Laser Therapy is the treatment of choice and can resolve most venous lakes in one or two treatment.

▪️The human scalp has 100,000 follicular units
▪️Each follicular unit has either one, two, three or four hairs growing from it. The average figure is 2-2.4
▪️Hair diameter and density declines with age.
▪️Normal hair density is 100 follicular units / cm2
▪️ Hair loss is only appreciable once at least half the hair in a particular area is lost
▪️ Hair follicles are constantly cycling and go through the following phases. At any one time, approximately 85% of hairs are in anagen, 2% in catagen and 13% in telogen
