Unmasking Idiopathic Diaphragmatic Palsy: A Practical Diagnostic Approach

Idiopathic diaphragmatic palsy remains an under-recognised cause of unexplained breathlessness.

In our retrospective cohort across two UK centres, we identify a simple, clinically applicable diagnostic triad:

-> Orthopnoea
-> Normal lung imaging
-> Elevated respiratory muscle strength ratios (MEP/MIP or MEP/SNIP ≄2.7)

This approach offers a pragmatic pathway to diagnosis-particularly in patients unable to undergo conventional supine testing.

We also demonstrate that non-invasive ventilation (NIV) is associated with meaningful improvements in lung function and symptoms over time, reinforcing its role in both management and prognosis.

Early recognition is key.

Now sitting on the other side of the table as an MRCP PACES Examiner…

Not everything in medicine is tested by how much you know…�Some of it is tested by how much you can be trusted.

PACES isn’t about memorizing answers -ļæ½it’s about thinking clearly, communicating confidently, and putting the patient first.

Because at the end of the day, the real question is:ļæ½ ā€œWould I trust this doctor with my patient?ā€

If you’re preparing, focus less on perfection - and more on clarity, structure, and calm decision-making.

I’m pleased to share that our research project, ā€œTranscutaneous COā‚‚ Monitoring Versus Arterial PaCOā‚‚ During Acute Non-Invasive Ventilation in Acute Hypercapnic Respiratory Failure: A Systematic Review and Protocol for a Randomised Controlled Trial,ā€ has been accepted for presentation at the American Thoracic Society International Conference 2026 in Orlando.

This work explores the role of transcutaneous carbon dioxide (TcCOā‚‚) monitoring as a non-invasive alternative to arterial blood gas analysis for guiding acute non-invasive ventilation in patients with hypercapnic respiratory failure. By combining a systematic review with the development of a randomised controlled trial protocol, the project aims to support more continuous, patient-centred physiological monitoring during acute NIV.
