Blackpool Teaching Hospitals: Alternatives to hospital by

The Trust has shared the below information to support patients with where to go for the most appropriate care 👇

Care Coordination hub;
Care coordination is a wide network of expertise that is accessed through a single point of access. One referral into the hub can trigger multi-organisation care.
This is for residents who are at risk of hospital admission and need to be seen the same day. For more routine care please contact their GP.
The hub is managed by a multidisciplinary team made up of professionals from FCMS, Blackpool Teaching Hospitals Rapid Response team and Primary Care.

Community nursing and therapy services;
If the patient is under the care of a district nurse or community therapy please contact the community Blackpool Teaching Hospitals centralised administrative hub.
Referrals can be made between 7.30am – 10pm seven days a week via email bfwh.community.adults.nursingandtherapy@nhs.net or calling the team.

Virtual wards
Virtual wards provide consultant led care in the patient’s usual place of residence to avoid admission or support early transfer out from an acute inpatient bed. Care can be delivered face-to-face or through remote clinical monitoring technology.

We operate the following specialist virtual wards:
• General Virtual Ward
• Frailty Virtual Ward
• Palliative End of Life virtual Ward
• Paediatric Virtual Ward
• IV Therapy Virtual Ward
