Our close-knit team of GPs, nurses, allied health professionals, administrative and reception staff, are dedicated to providing the best possible healthcare services for our community. Our team works collaboratively to provide healthcare services at The Hereward Practice. We are fortunate to have a close-knit team of GPs, nurses, allied health professionals, administrative and reception staff, who are all dedicated to providing the best possible service for our community. Patients are at the heart of what we do and we couldn’t do it without our extensive team. We are lucky to have so many experienced and long serving colleagues, plus newer staff who are committed to a career in Primary Care. We have an experienced management team who deal with the daily running of our practices and our patient service advisors are there to help whether you have a medical concern or administrative query. They are very busy so if your enquiry can wait until after 11am each day then that would be appreciated and should save you time either waiting on the telephone or in our reception areas. We engage regularly with patients through our Patient Participation Group, taking their views into account where possible when making changes and listening to their views, issues and ideas. We also have productive working relationships with the Integrated Care Board and Care Quality Commission and make improvements as and where we can. As well as our daily work in surgeries we also work in the local community, making home visits, providing healthcare for residents in local care homes and offer advice and support on lifestyle and non-clinical issues with our Personalised Care Team of care coordinators and social prescribers. We are busier than ever so thank you for your understanding while we continue recruit, train and develop our team and working practices to see as many patients as we can safely. The Hereward Practice is a part of Lakeside Healthcare, a large GP partnership that provides NHS care for around 170,000 patients across Lincolnshire, Cambridgeshire, Peterborough and Northamptonshire. We are made up of eight GP practices across 14 sites that are all part of their local communities. We work together as practices to provide resilience, sharing learning and professional back-office support services. Our most important asset is our people, who provide care for our patients and help run our surgeries smoothly. We employ over 500 clinical and non-clinical staff and we invest in training and developing to ensure that we retain and attract good quality staff who want to stay with us and be part of our team. We also host a clinical research team who run high quality clinical trials and studies to support the development of new medications and preventative treatment for long-term conditions. The Bourne Autistic Adult Support Group is a friendly, supportive space for autistic adults to meet others and access support locally.

The group runs on the 2nd and 4th Monday of each month, from 11am to 1pm at Wake House in Bourne.

It’s organised by MindSpace and supported by a community connector with an understanding of autism and neurodiversity.

Last week, our Social Prescribing team marked Social Prescribing Day by setting up a stand in our practice, speaking with patients about the social prescribing support available.

Patients could stop for a chat, ask questions, pick up leaflets and take part in a few games, while finding out what social prescribing is and how it could help them.

There were some good conversations, from patients looking for stop smoking support to those managing long-term conditions. Some had already used the service and said it had really helped, while others said they hadn’t heard of it before and would consider using it in the future.

We spoke to around 80 patients during our recent Pharmacy First drop-in session at the practice.

The session gave patients the opportunity to find out how their local pharmacy can help treat common conditions. The NHS Pharmacy First scheme makes it easier for patients to access care quickly and conveniently. Trained pharmacists can provide advice and treatment for a range of common conditions, helping people get the support they need.

Mel Scott, Digital Transformation Manager for Four Counties PCN, and Stuart Hellon from Community Pharmacy Lincolnshire were there on the day to chat with patients, answer questions, and explain how the scheme works.

Many patients were interested to learn how quickly they can be seen and the range of conditions pharmacists can treat. Feedback was very positive, with many saying they would feel confident using Pharmacy First in the future. Others had already used the service and were impressed with the care they received.

Mel Scott said: “Events like this are a great way to connect with patients and raise awareness of Pharmacy First. Many people are surprised by the range of support available through their local pharmacy. It’s about making access to care simpler, quicker, and more convenient for everyone.”

Not sure about your vaccination status? Here’s how to check...

If you're unsure whether you’ve had your recommended vaccinations, you can check online using the NHS App or SystmOnline. Your vaccination history is usually listed in your online GP record.

Log in to the NHS App or SystmOnline and look under Your health record. Some older vaccinations may not appear online depending on when online record access was enabled. If anything appears to be missing or doesn’t look right, please get in touch and we’ll review it for you.

If you can’t access the NHS App or SystmOnline, you can contact us using Anima by submitting an administrative request. Our team will review your record and let you know what we find.

If you’re unable to use online services, please call the practice. If possible, calling in the afternoon can help as our phone lines are usually less busy. A member of the team can check your vaccination history and confirm your status.

If we find that you haven’t received the recommended vaccines, or if your record is unclear, we’ll discuss this with you and arrange any catch-up vaccinations you may need.
