About the Role
We are seeking a compassionate, organized, and proactive Care Coordinator to join our team. The successful candidate will play a key role in ensuring clients receive high-quality, person-cantered care by coordinating services, supporting care plans, and liaising with healthcare professionals, families, and community resources.

Key Responsibilities
- Assess client needs and develop individualized care plans
- Coordinate services with healthcare providers, social workers, and community agencies
- Monitor and evaluate care plans to ensure client goals are met
- Advocate for clients and provide ongoing support
- Maintain accurate documentation and case records
- Ensure compliance with relevant policies, regulations, and standards
- Communicate effectively with clients, families, and multidisciplinary teams

Qualifications & Requirements
- NVQ 3 in Healthcare or related field
- Proven experience in care coordination, case management, or healthcare support
- Strong knowledge of community resources and healthcare systems
- Excellent communication and organizational skills
- Ability to manage multiple cases effectively
- Proficiency in MS Office and electronic health record systems
- Valid driver’s license (A must have)

CARE COORDINATOR POSITION
Salary: £ 22,000 - £24,000
Location: Bournemouth/Moordown.

Working hours: 09.00 to 17:00 Monday to Friday + Weekend availability + holding the on-call on a rota basis at an additional pay.

May be involved in early starts when required.

Type of Work: Full-Time 40 hours a week

Must have a full driving license and access to own vehicle.

We are looking for a full-time Care Coordinator to join our friendly team at Adonai Care Services Ltd, Moordown office.
You will be joining our current coordinators in providing a high-quality service to our clients and staff.

Must have care experience. Ideally care co-ordinator experience, but not essential

Criteria
Experienced in scheduling/coordinating/rota planning in the care sector
Experienced using a rostering system
Strong work ethic, organisation skills, and positive personality
Experienced in care calls
Take part in holding the ā€˜out of hours’ on-call phone

Skills and experience you should have:

Good literacy skills.
Good presentation skills
Computer literate
Administrative experience
Full driving licence

We are recruiting healthcare assistants in the Dorset area, temporary and permanent roles. Shifts available are either nights or days - we are very flexible.
