Job Role: Care Coordinator
Location: Yeovil
Permanent: Full Time

This is a great opportunity for you to work for a growing Health Care Agency. The role is office based in Yeovil. A chance to make a difference to our clients across the Southwest of England, ensuring that all parties are safeguarded and protected from abuse and harm at all times.

This role involves attending initial assessments, undertaking yearly reviews, producing and updating care and support plans. You will also have a key role in supervising staff and monitoring their personal development in delivering the best care. You will receive support and guidance from the rest of the office team. Working alongside health care professionals to meet client outcomes. You must have outstanding communication skills and able to work in a fast-paced working environment.

About you:
• Attention to detail and ability to professionally communicate with Clients and Care staff is an absolute requirement.
• Must have a clean Driving License.
• Previous experience within a Care Setting is preferable.
• Previous role as a Care Coordinator would be very desirable.
• Ideally qualified to Level 3 Health & Social Care or equivalent.
• Good level of IT proficiency is essential.
