Pain is not always caused by underlying structure or pathology. It is possible to experience neuroplastic pain.

Neuroplastic pain is chronic pain that occurs when the nervous system becomes overly sensitive, even when there is little or no ongoing tissue damage. The brain and spinal cord essentially learn the pain.

Neuroplastic pain is a type of persistent pain driven by adaptations in the nervous system.

The brain has the capacity to adapt and change. This can result in it strengthening pain pathways, making the nervous system more sensitive to pain signals. Your brain may then interpret normal sensory input from the body as dangerous, causing pain or other symptoms, even when there is no on-going tissue damage.

Posture research highlighted by Peter O’Sullivan et al (shared through Pain-ed.com) challenges the traditional belief that there is one “correct” posture that prevents pain.

Evidence shows that no single posture consistently predicts pain. One of the key messages is that the spine is strong and adaptable and common warnings to protect the spine are not necessary and can be counter-productive as they create fear.

Having my own experience of a serious injury has profoundly changed how I work as a physiotherapist. We are more than just a broken bone. A bone doesn’t exist in isolation — it is connected to muscles, tendons, ligaments, connective tissue, and nerves. But even then, we are more than the sum of our parts.

A broken bone — or any injury — can mean loss of function, loss of independence, fear, anxiety, frustration, pain. The list could go on. Being on the receiving end of care gave me a powerful insight into the experience of being a patient. It deepened my empathy, strengthened my listening, and reinforced my belief that rehabilitation is not just about healing tissue — it’s about restoring confidence, rebuilding trust in your body, and supporting the whole person through recovery.

If you’re navigating an injury or supporting someone who is, know that you don’t have to face it alone. Reach out, ask questions, and seek care that sees you as a whole person — not just a diagnosis. Your recovery is about more than healing; it’s about reclaiming your life.

“It’s adhesions.”

The modern-day scapegoat.

Back pain? Adhesions.
Shoulder pain? Adhesions.
Can’t touch your toes? Definitely adhesions.

Apparently we’re all just walking around glued together.

Let’s be clear.

True adhesions are fibrotic connections that form after surgery, significant trauma, or major inflammation. They are real. They show up in post-operative cases. They can restrict movement. Sometimes they even require surgical intervention.

What they are not is the automatic explanation for every sore neck or stiff hamstring.

Most people with musculoskeletal pain:

• Haven’t had surgery
• Haven’t had major tissue damage
• Haven’t had the level of inflammation needed to create dense scar binding

Yet they’ve been told their tissue is “stuck.”

If adhesions were as common as social media suggests, we’d all be moving like rusty robots.

Pain is far more often linked to:

– Load tolerance
– Reduced conditioning
– Stress
– Sleep
– Nervous system sensitivity
– Fear of movement

Not fascia glued together like PVA.

And here’s the uncomfortable truth:

You are not manually “breaking down” deep adhesions with your elbow.

Manual therapy can help.
It can reduce sensitivity.
It can make movement feel easier.
It can change pain.

But that’s not the same as mechanically ripping apart scar tissue every session.

We don’t need dramatic tissue explanations to justify what we do.

Adhesions exist.

They’re just not the default cause of pain.
