Selective Mutism is often more than just anxiety around talking. There can also be a mixture of Autism, SM and PDA. This requires a good understanding of the nervous system and emotional safety and trust comes first. Progress takes time and small steps are important but ensuring the person is feeling safe and regulated at each stage. 🩷

Scrolling through FB today and reading some emails, I am constantly amazed at the lack of knowledge about where to start with supporting those with SM especially teenagers. Before we jump to “getting a teenager talking”, we need to ask a much more important question:

Do they feel safe enough to communicate at all?

One of the biggest misunderstandings around selective mutism is the assumption that silence is simply a lack of speaking behaviour that can be “worked on” through prompting, encouragement, rewards, speech practice, or direct elicitation.

But speech does not sit at the top of the pyramid.

It sits at the very end of a long chain of neurophysiological, emotional, sensory, relational, and communicative processes.

Many selectively mute teenagers are living in a state of chronic threat. Their nervous systems are not calmly choosing not to speak — they are protecting them. In that state, direct demands for speech can increase panic, shame, freezing, dissociation, and avoidance.

Before speech work comes:

• felt safety
• trust
• co-regulation
• autonomy
• reduction of performance pressure
• predictable interactions
• sensory safety
• relationships without hidden demands
• acceptance of all forms of communication
• reduced monitoring and scrutiny
• opportunities for success without speech
• nervous system regulation
• connection before expectation

And perhaps most importantly:

The young person needs repeated experiences of adults who are safe to be silent with.

Teenagers with selective mutism are often exhausted by years of being watched, prompted, praised for tiny verbal responses, discussed in front of others, or treated as communication projects rather than human beings.

Speech may emerge when safety emerges.

And sometimes progress looks like:
– staying in the room
– communicating nonverbally
– tolerating proximity
– laughing
– texting
– whispering to one trusted person
– showing personality through actions before words

Those things are not “nothing”.

They are foundations.

If we skip the foundations and go straight to eliciting speech, we risk building intervention on anxiety rather than connection.

Communication grows in safety, not surveillance.

🌟Most days I try my best to accommodate best appointment times to fit in around family and school commitments.
🌟I had a special school visit this morning and then a home visit later this afternoon, which meant 2 hours 'free' between visits and wouldn't make sense to travel home due to location. 🌟I do enjoy popping into a cafe for a nice coffee (and cake) and somewhere to write up my electronic notes and to catch up on some other admin tasks. The noise in the cafe (music, coffee machines, people talking) was a little too much for my ADHD brain which made me feel stressed after an hour, so decided to sit in the car and read - the lovely scenery is calming along with no noise!
🌟Sometimes we have to take ourselves out of a situation and think about what our nervous systems need...this is exactly the same for children and young people, especially those who are neurodivergent coping in noisy/ high demand environments. Often they need sensory/ movement breaks and some quiet time!
What happens if we push through this? Usually a dysregulated nervous system which is not optimal for learning!
🌟 As Parents we also need to ensure we are thinking about what our own nervous system needs, quick coffee break, walk, quiet room etc even if it's just for 5 mins!

🌟AAC is particularly useful for autistic children and young people. Even for children who can access spoken language, AAC can help support sentence structure and increased communication confidence and skills.

🌟All AAC needs good communication partner support and training.

🌟Often, it can be overwhelming knowing where to start and there are so many programmes available/ on the market. If you can, I recommend taking advantage of the discounts on AAC apps this April!

⭐️Thought it would be good to share my standard and detailed assessment packages and costs as this is the most common question I get asked on new enquiries.
👉Please note, an assessment is always required prior to being able to make the best clinical decisions on therapy (type of therapy, frequency, duration, location etc.) Although, a report can be very helpful for lots of reasons, I totally understand this isn't always required and if appropriate, we can look at a more bespoke assessment package.
💛Therapy sessions and costs are always discussed openly in the virtual follow-up if this is agreed.
🧩Costs may change, depending on location, complexity of assessment, individual circumstances and also comes down to the time required to complete the best clinical assessment possible for each client and their circle of support. A lot more goes on behind the scenes so it's not just the direct face-face time with a client/ family.
🫶Always happy to chat about any concerns and if I can help I will, otherwise will signpost to the best person e.g. clinical specialists.
⏰ Currently I am nearing capacity (didn't expect to be this busy after only setting up end of January) so there may be a wait for assessment/ therapy sessions, depending on location. I will always be transparent about my availability and clinical skills and signpost where possible.
