Here at Victoria Park Health & Wellbeing Hub we offer a free NHS Health Check ! 🩺❤️‍🩹

The aim of the NHS Health Check is to assess your risk of developing stroke,Heart Disease, Type 2 Diabetes and kidney disease in the next 10 years. 🩺

You are eligible for a free NHS Health Check if:
👉You are between 40-74 years old

👉You are registered to either Cannington Health Centre, Cranleigh Gardens Medical Centre, East Quay Medical Centre, North Petherton, Polden Medical Practice, Quantock, Redgate & Somerset Bridge Medical Centre, or Taunton Road Medical Centre.

👉You have not had an NHS Health Check within the last 5 years

👉You have NOT been diagnosed with or are NOT taking medication for heart disease (Including blood pressure medication),stroke,diabetes,chronic kidney disease, dementia or on a statin.

Did you know that you can request an appointment online through our website? This is available from 07:30am-6:30pm. You can do this by selecting the "contact us online" box on the home page of our website: https://www.canningtonhc.nhs.uk/ . Your request will come directly to reception and be put through to our clinicians to be triaged the same day.

Our phone lines can be extremely busy in the mornings so submitting an online request will allow you to bypass the telephone call queue. Alternatively if you are unable to access the internet you can still call reception and we will be very happy to help you on the phone.
