Emily is a specialist pelvic health physiotherapist treating women with pregnancy and postnatal issues, prolapse, bladder and bowel problems and perineal scar pain. Emily Krelle (MCSP, BSc Hons) is a Chartered Physiotherapist and is registered with the Health and Care Professions Council. She has been working in the specialist field of pelvic health physiotherapy since 2008 in both NHS and private hospitals in Bristol and Bath. Pelvic health problems include bladder and bowel issues (such as incontinence, urgency and difficult emptying), prolapse, and perineal scar pain. Although relatively common (1 in 3 women experience urinary incontinence in their lifetime, around 50% experience prolapse and 90% of women who have vaginal childbirth will tear to some degree) pelvic health issues often go unspoken whilst having a big impact on a woman's life. Traditionally pelvic floor exercises are promoted as treatment for these problems but for some women this can make symptoms worse. There is increasing awareness of the role of tension within the pelvic floor relating to these conditions. Emily assesses each woman holistically and checks for pelvic floor tension which is then treated using a range of techniques. Emily has two energetic young children and is a keen gardener and baker. A client this week told me that the nhs wait for pelvic health physiotherapy is currently 50 weeks in Bristol.

That means if referred today your appointment will be Thursday 28th August 2025.

With this in mind I thought I’d review the progress my clients have made on their PFDI-20 scores. (The PFDI-20 is a pelvic floor symptom impact questionnaire I use at the start of treatment, at follow ups and the end of treatment).

Here are the last 5 client scores (which I’ve completely anonymised).

It’s a really useful score. Often bringing up elements of pelvic function clients haven’t thought about.

Though it doesn’t show the human side- that one of these clients can now go out without worrying about where the nearest toilet is. Another can take part in an exercise class without needing a pad. Another can do a poo without pain and difficulty.

50 weeks is a long time to wait to make progress.

Understand, think, feel, effort

Change isn’t always easy. You’ll do better if you understand what needs to change, how to achieve it, think about how it feels to enable self assessment of your progress, and then keep putting in the effort.

I aim to coach my clients to achieve pelvic health, and lasting change in this way.

So here I am, putting the effort in to attempt to change my running style to that of a beautifully elastic Kenyan runner following an excellent running workshop from Shane Benzie (Author of The Lost Art of Running) CrossFit Henleaze last weekend.

Practise makes permanent.

Our bodies get used to what we do frequently. Are you sitting reading this? Legs crossed? Or legs swept to the side, curled up on the sofa 🛋️

Let’s think about what happens when you cross your legs. Right over left. Your right hip is probably now forwards compared to the left. The thigh is lifted off of the seat. Both legs are crossing your body’s midline.
That pelvis has definitely become a bit wonky, or asymmetrical.

If every time you sit you cross your legs your body will adapt to support you to do that. Muscles shorten, fascia becomes stiffer. We’ve made a body that’s great at sitting crossed legged.

When we then stand and move about we will still have those muscle and fascial changes and that will affect the way we move. It will create pull within the pelvis, areas of restriction, and impact the way our body transmits force that goes through it.

Within the pelvis that pull can result in a tissue shift such as a prolapse, or tissue restriction & incontinence.

The good news is that we can also retrain our movement. Set new demands for our bodies to get good at. Better ones. Iron out some of that asymmetry.
We have a relatively short time to work together in clinic. What you do the rest of the time matters.

I often post about pelvic floor issues but I also treat pregnancy related pains such as pelvic, back and rib pain. These pains can be debilitating and in the case of rib pain even make breathing sore!
