Home based care services are offered by many organisations, but our clients understand that Thornbury deliver a safe and flexible community service for people with the most complex care needs. For children and adults with long term conditions life outside a clinical institution will always carry a greater risk. Through our Community Services, we aim to reduce that risk to an acceptable level so that vulnerable people can step outside the ward or nursing home and either start or carry on living the life they want to live with the support of people they can trust. Through a combination of regional and local based staff, we are able set up packages throughout the UK. Our field based nurses regularly meet with Commissioners, LHB’s, Social Services, Case Managers, Agency Workers, Clients & their families. A Thornbury care team can be made up of Specialist Registered Nurses or Experienced Health Care Assistants, or even both. As people’s lives are unique so then will every care team be, and through a comprehensive assessment process we can match the right people to work with each individual client. We believe that offering the best possible service to our clients goes hand in hand with offering the best possible service to our agency workers, and so we want to reward them for the skills they have and the job they do.

Who we work with
Working with Clinical Commissioning Groups (CCGs), local authorities, case managers and private clients, we deliver tailored nursing care for both adults and children. We understand that everyone’s needs are different, our care packages can be tailored to whatever suits you best; from care around the clock, seven days a week, to just a couple of hours a day for a short period of time.
Located across England and Wales, we provide comprehensive nurse-led care packages for many highly complex and socially challenging patients. Our team is available 24-hours a day and has extensive experience in providing skilled nurses across a wide range of grades and specialties, quickly and effectively. As an established CQC registered care provider, we have a proven track record in preventing hospital admissions, even with many of our long-term clients being fully ventilated.
