- Mental Health Support

- Autism Support

- Buccal Midazolam

- Complex Care

- Epilepsy Support

Why CTS Matters

Far too often, people leaving hospitals face barriers to reintegration — from reliable transport to meeting their most urgent daily needs. Without the right support, these barriers become obstacles to lasting stability and recovery. At CTS, we remove those obstacles, empowering people to thrive in their communities with dignity, choice, and independence.

👉 If you are a commissioner, healthcare professional, or organisation committed to improving outcomes for people with multiple and complex needs, connect with us today to explore how we can make an impact together.

Discharge works best when people are prepared - not rushed.

CTS by Nurseline Healthare focuses on what happens before someone leaves hospital. We take the time to support people through change, rebuild confidence, and put the right foundations in place for life in the community.

By working closely with inpatient teams, commissioners, families, and community services, CTS helps reduce the risk of discharge breakdown and repeat admissions.

The result is more purposeful hospital stays, smoother transitions, and community placements that are set up to succeed long term.

If you’d like to explore how CTS could support an upcoming discharge, our team is always happy to talk. 👉https://hospitaltohome.co.uk/

From our CTS team, we’d like to wish you a peaceful holiday season. 🎄

This time of year is a reminder of how much care, steadiness, and feeling supported really matter - especially when routines change.

Wishing you calm moments and a gentle close to the year.

22/12/2025

The holidays are meant to be spent at home, in the community, with a bit of calm and familiarity.

Hospitals do play a vital role, but they are not where people build their lives.

When discharge is done properly, people can settle back into daily life and actually enjoy the time around them. Families aren’t left holding uncertainty, and the transition out of hospital feels considered rather than rushed.

Good discharge protects dignity, stability, and the chance to take part in everyday moments that matter - especially at this time of year.

As winter pressures rise, getting discharge right becomes even more important.

Because being home for the holidays only works when the journey there is handled well. 🏠✨

There’s a gap between being clinically ready and emotionally ready to leave hospital - and most people fall straight through it.

We step into that gap with a pathway of support that makes going home safe, smooth, and effective.

Because going home isn’t just a discharge.

It’s trust, confidence, and feeling safe enough to start again.

Our teams bring specialist skills - autism, learning disabilities, mental health, epilepsy, eating disorders, buccal midazolam, Makaton, complex care - and the compassion that helps people shape their own life beyond hospital.

Every transition home is a human rights moment, rooted in hope, choice, and dignity.

A hospital discharge can go two very different ways.

Without the right support?
It becomes leaving hospital… only to feel lost, overwhelmed, and at risk of going back.

With the right support?
It becomes confidence, familiarity, and a life someone doesn’t need to step away from again.

That’s why CTS exists.

Through our human-rights-based, trauma-informed PROACT-SCIPr-UK® practices, our PBS practitioners, expert support workers, and specialist teams work alongside people in hospital - r trust, routines, and the everyday skills that make returning to the community feel hopeful, not frightening.

Because a safe discharge is only the beginning.

The real goal is a life that feels stable, meaningful, and truly lived.

If you’re seeking reliable discharge support that truly lasts beyond hospital doors, our team is ready to help make that happen. - https://hospitaltohome.co.uk/

Leaving the hospital should feel like walking into comfort and safety, not into uncertainty and fear.

In this video, Jenny McGuire, our Senior Care Co-Ordinator, takes you behind the scenes of our Community Transition Service (CTS), showing how we bring people back home with care that’s personal, real, and filled with heart. It’s about more than just support – it’s about making people feel like they truly belong, respected, and cared for in their own space.

Here’s how we make a real difference:

✅ Care that revolves around the person – because no two journeys are the same.

✅ Tailored training for nurses and support workers – from our in-house PBS specialists and ProAct-SCIPr® champions, ensuring those who care for you are always equipped to give their best.

✅ Ongoing emotional and professional support – because we know carers need care too.

✅ A true homecoming – where care is built around the person, not the other way around.

Everyone deserves to live with dignity, trust, and a sense of belonging. It’s about feeling heard, feeling cherished, and thriving in the place they call home.

Being a support worker or a nurse is much more than just a job – it’s about showing compassion and creating connections that truly make a difference in someone’s life. 💙

In this video, our team members share the powerful journey of working with the people they support where person-centred care and empathy form the foundation of every interaction. The smiles, the warmth, and the hugs are all part of the human connection that makes this job unique.

Trust is built slowly, day by day, through love, patience, and understanding. When we approach each person with compassion, we offer them the respect, understanding, and support they need to grow and thrive.

This is why we do what we do – to offer care that’s rooted in love, trust, and respect. 💙
