We created this space to bring clarity to what our Community Transition Service (CTS) by Nurseline Healthcare does - and to show how it supports people move from hospital to home safely, confidently, and without delays.

Too often, people experience uncertainty when it’s time to leave hospital. CTS changes that.

Our teams work closely with the families - and the person we support-, healthcare professionals, clinicians, and local services to make each transition person-led, trauma-informed, and focused on what matters most - feeling at home again.
