As you may well know, the new EU legislation, GDPR, comes into force on 25th May 2018. This affects your data, how it is stored, how it can be used. And how you can access it. As the individual who has provided that data, you now own that data, rather than the business holding it.
What are your rights as the individual?

What data do I collect at Body Revive?
For the purposes of medical screening, to provide you with the best possible service and to fulfill insurance and legal obligations, I collect the following:
Personal information such as name, date of birth, contact details, lifestyle (i.e. exercise regime, dietary regime, drinking and smoking habits etc), doctor’s surgery contact details and medical history. Treatment details. This information is collected on paper forms. This won’t be changing in the foreseeable future.
The notes are stored securely for 7 years. If you don’t come and visit us for a treatment for 7 years, then these records will be shredded.
When you next visit the clinic, you will be provided with a copy of the data privacy policy and a new form to sign to say you not only consent to treatment, but also that you are happy for us to store your personal data, and will shred it after 7 years if you we are not treating you.
We do not do mailshots, promotions etc via email or text. When you next visit the clinic, you will be asked to sign a disclaimer to say you are happy for me to communicate with you via email, Facebook message or text.
We have never shared any information without the approval of the individual concerned, and has only been for referral or case study and only on signed agreement by the individual.
