In a world of domiciliary care, recognising when your client is unwell Is crucial. A key concept carers must be familiar with is frailty – a common condition among older adults and those with multiple chronic conditions. It is often presented in ways that are subtle and easily overlooked.
Understanding these signs and common presenting conditions associated with frailty can empower caregivers and family to provide timely and appropriate care.
Common presenting conditions in clients living with frailty:
1- Falls and mobility issues:
History of falls or a fear of falls can indicate underlying frailty. Factors include muscle weakness, poor balance, vision problems and medication that affects coordination.
2- Unintentional weight loss:
This may result from poor appetite, difficulty swallowing, malabsorption or increased metabolic demands due to chronic illnesses.
3- Fatigue and low energy:
This could be due to anaemia, heart failure, chronic infections or a general decline in physical function.
4- Cognitive decline and delirium:
Delirium, an acute state of confusion, can be triggered by infections, medications, dehydration or changes in the environment.
5- Chronic pain:
Conditions such as arthritis, osteoporosis and neuropathy can cause persistent pain that affects mobility and quality of life.
6- Polypharmacy:
Many clients living with frailty take multiple medications for various medical conditions. The interaction between these medications can lead to adverse effects such as dizziness, confusion and falls.
7- Frequent infections:
Clients living with frailty often have weakened immune systems, making them more prone to infections, particularly respiratory and urinary tract infections.

Taking action:
Caregivers should –
1- Conduct regular assessments to monitor for signs of increasing frailty and associated conditions.
2- Maintain open communication with clients, their families and healthcare providers to share observations and concerns.
3- Create and follow personalised care plans that address the specific needs of clients living with frailty.
4- Encourage and support activities and lifestyle choices, such as balanced nutrition, regular physical activity and social engagement to improve overall well-being.
