Here’s how to submit your requests 👇

💻 Online: Use our online form on our website https://accurx.nhs.uk/patient-initiated/m83027 to tell us about your medical or admin request.
📞 By phone: Call us on 01283 845555 if you can’t complete the form online — our team will help you fill it in.
🏥 In person: You can still come to reception, and our staff will help you submit your request using the same system.

Once your request is received, our clinical team will review it and let you know the next steps — whether that’s a same-day appointment, a phone call, or advice from another healthcare professional.
