At this surgery we have a Social Prescriber and a Health and Wellbeing Coach. They will work together with you to:
• Explore what is important for your life and wellbeing
• Identify the local activities and services you can benefit from
• Will help you to gain confidence to start using services that can help you

How do I access the service?
It is simple, you can contact the surgery and ask to talk to your Social Prescriber. You can talk to your GP, healthcare professional or surgery staff and our Social Prescriber will get in touch with you.

Who is it for?
It is available to everyone.

Does it cost?
No, the support we provide is free. You only need to invest your time to make changes to improve your health and sense of wellbeing.
