Care Assessor – Ely and Cambridge

We are seeking a dedicated and experienced Care Assessor to join our team in Ely, working to ensure the highest standards of care for individuals in their own homes.

About the Role:

As a Care Assessor, you will be responsible for:

· Conducting thorough assessments of individuals' needs and preferences, ensuring they are at the centre of their care plan.
· Developing and implementing individualized care plans that promote independence and well-being.
· Working collaboratively with individuals, their families, and other professionals to provide effective and person-centred care.
· Ensuring compliance with relevant regulations and standards.
· Providing ongoing support and guidance to care staff.

· Take part in the On call rota.

About You:

· Experience in adult social care and a solid understanding of care standards and best practice.
· Excellent communication, interpersonal, and problem-solving skills.
· Ability to work independently and as part of a team.
· A passion for supporting vulnerable individuals and promoting their independence.
· A full driving license is essential.
