So divine meeting a like-minded pelvic health physio from Cape Town. We’ve never met in person, but I’ve sent many of you her way over the years. It felt like catching up with an old friend over a coffee but that friend also champions squatty potties in her past time, has no concept of TMI and could probably tell you more than you need to know about l**e.

Having emigrated twice and had a baby this year, I’ve seriously missed tapping into my physio brain.

Conversations and connections like this remind me why I love this work so deeply because it’s not just about anatomy and muscles, it’s about humanity, connection and the joy of breaking taboos that desperately need breaking. Pelvic health is such a vital part of our wellbeing and sharing space with women who are equally dedicated to lifting that conversation higher leaves me inspired.

Postpartum recovery requires more than time away from work (we all know that this is no restful vacation).
It requires time, support and the security to heal (mentally and physically), move safely and re-establish strength.

When maternity leave is doused with stress, short or unpaid, many parents miss the opportunity to seek help in this critical window, increasing the risk of persistent symptoms relating to; incontinence, prolapse, pain etc.

Fair paid leave = pelvic health

The significance of this transition deserves to be acknowledged by leaders and reflected in company policies (despite the position that government may hold).
It’s very interesting to not see this acknowledgment reflected in the policies of wellness and health care facilities that advocate for the comprehensive health of their paying customers…

Sleep and Intimacy

Research shows that just 1 extra hour of sleep can boost the likelihood of partnered intimacy the next day by 14% (Kalmbach et al. 2015)

How does sleep impact s.

Estrogen, testosterone and progesterone levels are influenced by sleep and play a role in desire, arousal, receptivity, sensitivity, tissue health

S. Drive - better known as s. motivation may be impacted by poorly regulated stress hormones and a muted dopamine response

REM sleep preserves local spontaneous nocturnal blood flow (tumescence), receptivity and response to cues and general energy for s. activity.

Postpartum note: Sleep is especially disrupted in the early months after childbirth. Combined with hormonal shifts (decr. estrogen/progesterone, incr. prolactin), this can contribute to feelings of low motivation, dryness & fatigue. This can be rather distressing leading to a lot of self blame. Considering the significant impact of sleep (described above), change in lifestyle and hormonal shifts - I hope that this helps you to reduce the pressure and expectation a little in this transitional time!

Sleep isn’t “just rest.” It’s essential for wellbeing, s. connection and s. health.

A note: the study above shows correlation not causation and focuses on young women so may not generalize to the broader population. Nevertheless the findings are interesting!

Quite aptly… I missed it, because I’m feeding all the time.

This journey has been a beautifully challenging paradox.
We’re currently navigating the curveball of an elimination diet to uncover the culprit behind a cluster of allergy symptoms. If you’re curious, search Picariello et al. (2019) for research on peptides in breastmilk and clearing times.

Our current rhythm:
• One expressed bottle each morning so I can grab a sliver of sleep, then pump
• One sleepy little feed around 3ish AM
• A perfect latch… sometimes 😵‍💫other times, we’re learning through together.
• And then THAT moment: when she pulls off after comfort feeding, I melt 🥹 ( a lot of non nutritive suckling at the moment with a developmental leap and her eczema discomfort).
• She gave us a 7 hour stretch last night! From 10:15 - 5:08 am! It was glorious but she didn’t sleep and fussed the entire day yesterday.. 🤔

There is probably more but I’ve forgotten because this mama is tired 🥱

Requests + recommendations needed please:
• I need more nursing bras: the once great H&M ones have changed their pattern and are now just meh. What are your favourites with clippies that are available in South Africa? 🇿🇦
• I’ve had to stop lanolin due to Charli’s eczema. Any tried and trusted alternatives you’d recommend?

My first solo trip out the house with Charli 🫶

This am we went to the event
Charlotte pooped once, fed twice (in the spirit of my nips we’re out more than they were in) and demanded all my attention 😅 but I had a willing baby sitter while I ate, met another mom who recently immigrated to the uk, chatted to Tami - a sleep consultant ( who 1. Reassured me that we are on the right track and 2.gave me a good tip to help transition Charlotte from over my shoulder and into the crib), yapped over a good coffee, and I even got in a mini photoshoot with

An all round really positive morning out!

It’s definitely broken the ice for more trips out the house for the two of us!

I’ve had quite a few questions about Charli’s birth story, some gentle, some not.
There’s so much I want to say, but the words are still hard to find.

As a pelvic health physio, I work closely with expecting and postpartum parents. I know how common birth trauma is, too common. Sometimes it’s physical, sometimes mental, and often both.

While I’m not ready to share our story just yet, I do want to speak up this week to highlight the prevalence of traumatic birth experiences and advocate for better maternal care.

A few stats:
Up to 1 in 3 women report their birth as traumatic.
15–20% of those may go on to develop PTSD, possibly experiencing flashbacks, anxiety, or intrusive thoughts.

Other related mental health conditions include:
• Postpartum depression (PND)
• Postpartum anxiety
• Tokophobia (fear of childbirth)

This year’s theme “inequality” reminds us that these stats likely underrepresent the real numbers due to a lack of routine screening.

Additionally it is improtant to acknowledge that rates of trauma and mental health challenges are even higher among certain groups, including by race, age, and socioeconomic status.

If any of this sounds familiar, please know you are not alone. You deserve care, support, and healing. 🫶

In a culture that encourages bouncing back and concealing any trace of what was once a pregnant body, I didn’t expect to feel so tied to what was once your home.

My belly is going down, I can roll in bed without a three-point turn, and I can see my toes again. These are good things, signs that I’m coming back to myself, that my body is healing. But my body still feels like your body, and I’m not quite ready to let go of that.

I find myself resting my hand on my soft belly, in awe, just like I used to. In these moments, I feel closer to you. I’m reminded of the most magical and transformative thing my body will ever do, not just growing you, but becoming your first home.

This belly, this softness, tethers me to you, to the time we were one. The world might try rush us toward “after,” but I’m going to lingering here a little longer. In the in-between.

Early am cuddle in bed before a feed!

At this point the I am sopping wet from let down (do you sleep on a linen saver? It sounds hot. I can’t imagine sleeping with a bra on! Help a girl out here because I feel like we are going to be washing daily!?)
And the right is an absolute beast of a b**b 😅
Even though it’s not the right b**bs turn, it’s always trying to take over the show

Did you know that breast / chest feeding or pumping has the potential to impact your vulvar symptoms?

If you are experiencing any of the following symptoms, it’s worth enquiring and looking into GSL (genitourinary syndrome of lactation)

Dryness
Delayed perineal healing
Pain / dyspareunia - superficially or deeper (this is a common presentation I see in practice)
Decreased s..ual satisfaction
Urinary frequency / urgency / leaking

🔔 These symptoms might ring a bell?
Interestingly, since symptoms result from a change in your hormonal milieu, (increased prolactin ➡️ decr GnRH DECR LH &FSH ➡️ decr estradiol ) GSL presents similarly to GSM (genitourinary syndrome of menopause)!

1 week of Charli

Introducing the not so little babe that made us parents 💕

Charlotte Roe Langford (Charli for short) arrived on the 23rd of June, 41+4, a chunky 4.3kg’s and we are madly in love!

A long and complicated series of events brought her into our arms and we have some difficult memories to process, in good time. For now, I am recovering, we are taking it slow, allowing ourselves to feel the challenging feelings and just loving our little bubble together!

This weeks recovery wins:
I can pick up and hold Charli solo.
I can now do a night feed solo.
Slowly weaning off pain meds.
I wore pants for the first time yesterday.
Today, I managed to touch my feet for the first time and got in a 1,5km very slow walk this am (+midway feed in the park of course, oh and a bird 💩 on my head 👌).
Every other moment has been spent logging dirty diapers, wet diapers, naps & right / left feeds… ☺️

In awe of our bodies and more in love with than ever before ❤️

40 week Routine

You will notice there are only 2 strength elements now and the rest is basically mobility. This heavy body and belly gets tired quick … also, considering I could go into labour at any moment 🤯 I don’t want to be fatigued!

1. Step up - targeting deep rotators (& balance / core). Great option for preventing pregnancy related pelvic girdle pain (PPGP) or (like me) management after resolving sij pain. See previous reels. Advice should NOT be “wait until baby arrives”, impacting 20-70% of pregnant individuals, and leading to depression in 24%. Management of PPGP is important! 📚PMID: 39144405.

2. Single leg Romanian dead lift, I’ve dropped to 6kg’s, previously 12kg. Great for hamstrings, posterior pelvic floor and pelvic position & PGP, the shoulder press makes it full body, incorporating core and upper body. Hold on to a wall if needed.

3. Getting into a little thoracic rotation. You can use this maneuver on the toilet for constipation too.

4. Extension rotation! I have done this one a lot!! One of my favorites for mid / upper back tension in pregnancy - you know that tension you get right around the level of your bra strap that is so tough to target!!

5. Working on internal and external hip rotation. I think I saw this on Vernon’s page - tagged in slide.

6. Rotating pelvis over femur at 90’ degrees hip flexion. Maintaining strength and control through hip rotation range. Good for mid pelvis mobility for delivery.

Note on 1. 2. & 6.
I am a firm believer that we maintain range, specifically control through range by strengthening through range. For this reason I do not tell pregnant patients to stop pelvic floor exercises in the third trimester, rather we maintain control and awareness throughout range of motion. As opposed to suddenly switching to stretches only where there is a chance of reduced awareness and control through range. Similarly, if hips are tight, we can focus on improving control through inner and outer ranges as opposed to defaulting to stretches and release only.

I was hoping that this would trigger labour… at 40+1 but this baby is super comfy 😅
