What does brain fog feel like in perimenopause?

Many women describe brain fog as forgetfulness.

But in reality, it often feels much more unsettling than that.

For me, it felt like my brain was full of cotton wool.

My thinking was slower.
I struggled to concentrate.
Information just didn’t stick.

I would read something and realise I hadn’t taken any of it in.
I started forgetting things at work which increased my anxiety.

There was also a sense of disconnection.

Not confusion, but a lack of clarity.

Like I wasn’t as mentally present as I used to be.

At times, it was frightening.
I even worried about early dementia.

Common brain fog symptoms in perimenopause include:

• Memory lapses
• Poor concentration
• Word-finding difficulties
• Mental fatigue
• Feeling overwhelmed by simple tasks

These symptoms often fluctuate and may worsen with stress, poor sleep or hormonal changes.

A few years ago I genuinely wondered if I was developing dementia.

Not during the day.
During the night.

You know that time when you wake up at 3am and your brain decides to review every possible worst-case scenario?

I’d lie there thinking about all the things I’d forgotten recently.

Walking into rooms and not remembering why.

Losing my train of thought halfway through a sentence.

Forgetting simple words.

So I did what many sensible people do at 3am.

I started looking up dementia symptoms online.

I even found a few questionnaires and did those.

Apparently I passed.

Which was reassuring.

But it didn’t explain why my brain suddenly felt like it had been wrapped in cotton wool.

Later I realised what was actually going on.

Perimenopause.

Brain fog is incredibly common during this stage of life, but when it first happens it can be quite unsettling.

Especially if you’ve always relied on your brain at work.

Something I see a lot with women in midlife is worry about ageing parents.

It’s rarely dramatic.
It’s more of a quiet background concern that never really switches off.

You notice small changes.
You start thinking ahead.
And suddenly part of your mind is always half on.

This often sits alongside everything else perimenopause brings.

Poor sleep.
Less tolerance for stress.
Feeling more on edge than you used to.

What’s interesting is that this kind of worry isn’t really about the future.

It’s about what your nervous system is doing now.

Holding responsibility in two directions keeps the system alert.

If your hormones are already shifting, that alertness can tip into anxiety much more easily.
