🦿 PRE OPERATION 🦿

Pre operation physiotherapy or “prehabilitation” are critical components in preparing patients for Total Knee Replacement (TKR) surgery. These interventions aim to reduce post-operative pain, optimize functional outcomes and facilitate a faster recovery.

PAIN MANAGEMENT & EDUCATION
One of the key goals of pre-operative physiotherapy is to minimize post-operative pain and complications such as joint stiffness. Pain management strategies, including range of movement exercises and neuromuscular strength control, are integral to ensuring smoother post-operative recovery.

In addition to physical benefits, pre-operative rehabilitation offers psychological benefits, helping to reduce anxiety and improve patient confidence before surgery. Patient education, which is often included in prehabilitation programs, informs patients about the surgery, rehabilitation process, and potential challenges they may face post-operatively.

RANGE OF MOVEMENT & STRENGTH
Pre-operative physiotherapy focuses on promoting range of movement as pain allows and strengthening the muscles around the knee joint, particularly the quadriceps and hamstrings, which are key in knee function and stability. Poor range of movement and reduced muscle strength can lead to increased joint stress post-operatively, which can slow recovery.

FUNCTIONAL RECOVERY
Prehabilitation also contributes to improved functional recovery, which can result in a shorter hospital stay. Faster mobilization post-surgery reduces the risk of complications such as deep vein thrombosis (DVT) and infections.

Return to Sport Stages after ACL rehabilitation are crucial to ensure a gradual, safe, and effective transition back to the demands of athletic competition.

- Jogging
Jogging is typically the first significant activity after regaining strength and stability. It helps reintroduce weight-bearing and dynamic movement to the knee in a controlled manner.

- High-Speed Running
Once jogging is tolerated, the next step is to increase running speed to prepare the athlete for the higher demands of sport specific movements.

- Change of Direction Drills
Cutting, pivoting, and lateral movements place additional stress on the ACL and are critical to most sports.

- Maximal Speed Running
Running at maximal speed is essential for athletes in sports that involve sprinting. It introduces maximal forces on the knee joint and prepares the athlete for high-intensity play.

- Football Positional Drills
This phase involves integrating sport-specific positional drills, which mimic in-game scenarios, but under controlled, non-contact conditions.

- Non Contact Training
Non contact training replicates full practice sessions but excludes contact elements. It allows the athlete to experience the dynamic environment of a game while minimizing injury risk.

- Full Contact Training
This stage involves reintroducing the athlete to full contact situations typical of competitive play, such as tackles or physical challenges.

- Training Load
Gradual increase in training load allows the athlete to tolerate the physical demands of full training sessions and games without overloading the knee.

- Match Volumes
Gradual exposure to match volumes, starting with limited playing time and progressively increasing toward full games.

Getting back to sport from serious injury can be one of the toughest challenges for an athlete both physically and mentally.

Having had my own injuries during my career, I can empathise and relate to each stage of my patients recovery journey because I’ve been there myself.

- ACL injury March 2008
- Second ACL and meniscus injury December 2008
- Second meniscus injury January 2012

📊 TESTING BATTERY 📈

The importance of a battery of testing in ACL rehabilitation and return to sports is well-supported by research, as it provides a comprehensive and objective evaluation of an athlete’s recovery.

Returning to sports prematurely after ACL reconstruction is a significant risk factor for re-injury, particularly for young athletes. Wiggins et al. (2016) found that athletes under the age of 25 who return to sport without meeting specific return-to-sport (RTS) criteria have a higher risk of reinjury to either the reconstructed or contralateral ACL. A battery of tests, including strength, functional tests and patient-reported outcome measures, helps to ensure that athletes only return to sport when they are physically and mentally ready, reducing the likelihood of re-injury.

A return to sport testing battery should include:

- Strength Testing
- Hop Testing
- Pitch Testing
- Patient-Reported Outcome Measures

Sports specific training is crucial in ACL rehabilitation because it tailors the recovery process to meet the unique demands of the athlete’s sport, helping to bridge the gap between basic rehabilitation and full athletic performance.

Here are 5 key reasons why sports-specific training is important in ACL rehabilitation:

- Sport Specific Movements: Sports specific training incorporates movements, skills, and patterns that are directly related to the athlete’s sport, such as cutting, pivoting, jumping, and landing. This helps retrain the body and the nervous system to perform these movements safely, reducing the risk of re-injury.

- Strength and Conditioning: Different sports require different types of strength, endurance, and agility. For example, a football player may need more lateral agility, while a basketball player may require more vertical power. Sports-specific training ensures that the athlete builds the exact type of strength and conditioning needed for optimal performance in their specific sport.

⚡️ AGILITY ⚡️

Agility in sport is defined as ‘‘a rapid whole-body movement with change of velocity or direction in response to a stimulus’’

Sometimes used interchangeably with change of direction in ACL Rehabilitation and S&C, Agility requires the athlete to use the ability to react in an unpredictable sporting environment.

This skill involves a combination of perception, decision making, coordination, change of direction speed, reactive strength and timing. Agility is crucial in many sports as it enables athletes to react promptly to the dynamic and unpredictable nature of gameplay to support return to sport following ACL Reconstruction.

REACTIVE AGILITY TEST
The Reactive Agility Test (RAT) is an advanced measure of agility that not only assesses an individual’s ability to change direction quickly but also incorporates their reaction to unpredictable stimuli. This makes it a more comprehensive evaluation of functional agility in real-world sports scenarios. The RAT involves an athlete reacting to a visual or auditory stimulus to change direction or perform a specific movement as quickly as possible.

Change of direction (COD) is any activity that involves a rapid whole-body movement with a pre-planned change of velocity or direction.

Incorporating change of direction training into ACL rehabilitation programs is essential for achieving optimal recovery and ensuring a safe return to sports and daily activities. COD training addresses key aspects such as neuromuscular control, muscle strength, and the ability to perform multi directional/sport-specific movements, all of which are crucial for reducing the risk of re-injury and enhancing overall knee function.

Key components in change of direction include:
- Acceleration/Deceleration
- Maneuverability

ACCELERATION/DECELERATION
In sports, successful COD movements require the seamless integration of acceleration and deceleration. Athletes must be able to accelerate quickly to reach their top speed and decelerate effectively to control their movements when changing direction. Training programs often focus on drills that combine these elements to simulate game situations and improve overall athletic performance such as the 505 test and drill.

MANEUVERABILITY
Maneuverability in COD movements in sports refers to an athlete’s ability to efficiently and effectively change their body position and direction while maintaining control, speed, and balance. The Illinois Agility Test and drill is a widely used and validated outcome measure for assessing change of direction and maneuverability training.
