Care Coordinator (Health & Social Care)
Location: Southend on sea (SS1)
Salary: £14ph
Job Type: Permanent
About the Role
We are looking for a compassionate and organised Care Coordinator to join our team. You will play a key role in ensuring high-quality care is delivered by coordinating care workers, managing schedules, and supporting clients to live independently.
Key Responsibilities
• Coordinate and manage care staff rotas and schedules
• Liaise with clients, families, and healthcare professionals
• Conduct care assessments and reviews
• Ensure care plans are up to date and meet individual needs
• Monitor service quality and compliance with regulations
• Handle day-to-day enquiries and resolve issues promptly
Requirements
• Experience in health and social care
• Strong organisational and communication skills
• Ability to work under pressure and manage multiple tasks
• Knowledge of care standards and safeguarding practices
• IT skills (e.g., care management systems, Microsoft Office)
• NVQ Level 3 in Health & Social Care
• Driving licence and access to own car
