🚑 Join Our Hospital Discharge & Reablement Team 🚑

Are you passionate about making a difference in people’s lives at one of their most important moments?
We’re looking for caring, resilient, and proactive individuals to join our Hospital Discharge Team – supporting patients to safely transition from hospital back into their own homes.

About the Role:

Must be a driver – however, we will also consider a non-driver with the right attitude, availability, and passion for helping others.

Must be able to communicate effectively across all disciplines, including hospital staff, community teams, clients, and families.

Based in the community, you will be the first face a client sees when returning home from hospital.

Your role will be to facilitate safe discharges, recognising when it may be unsafe for someone to return home and taking the right action.

Support clients in settling back into home life, offering reablement and confidence-building to help them regain independence.

We’re Looking For:

A compassionate and understanding nature

The ability to remain calm and resilient in challenging situations

Strong communication skills to work collaboratively across healthcare and support networks

A genuine passion for improving quality of life for others
