Bank Community Support Workers – Derbyshire

Peak Home Care is looking for compassionate and reliable Bank Community Support Workers to join our growing team, supporting individuals to live independently within their own homes and communities.

This is a flexible, bank role ideal for those seeking work that fits around other commitments while making a real difference to people’s lives.

Role Responsibilities

Providing person-centred care and support in the community

Assisting with daily living tasks such as personal care, meal preparation, and medication support

Supporting individuals to access their local community and maintain independence

Promoting dignity, choice, and wellbeing at all times

Accurately completing care records and following care plans

Caring, patient, and reliable individuals

Good communication and interpersonal skills

Based in the community, you will be the first face a client sees when returning home from hospital.

Your role will be to facilitate safe discharges, recognising when it may be unsafe for someone to return home and taking the right action.

Support clients in settling back into home life, offering reablement and confidence-building to help them regain independence.
