Chronic low self-esteem is not merely a collection of negative thoughts or surface-level insecurities, it may be the visible tip of a deeper, imperfect shame-based emotional system. Often hidden beneath symptoms of anxiety or depression, shame fuels emotional dysregulation, self-sabotaging behaviours, and entrenched beliefs of unworthiness. NeuroAffective-CBT lends itself to the online clinical environment as it is clearly demostrated in this role-play with psychotherapist Carla Vercruysse. I coined the term 'empathic-mentalisation' to describe how a therapist does not only understand at a cognitive level patient's distress and vulnerability but also aims to 'feel' client's internal struggles through compassionate-imagination and personal experience of distress. Being practical and goal-oriented should not come at the expense of a compassionate dialogue. But how could this be conveyed through language..? In this brief video, Daniel Mirea (therapist's role) elicits important risk factors: sleeping issues, a poor diet alongside lack of exercise and physical activity. This is in line with the 'TED' model or 'TED's your best friend' approach which would eventually be introduced to the patient in parallel with tailored psychoeducation.

NA-CBT... the new CBT therapy

I am an accredited CBT Psychotherapist, a writer, senior lecturer and clinical supervisor who trained and also trains in different cognitive and behavioural approaches including traditional CBT, Compassion Focused Therapy, Cognitive-Behavioural Hypnosis, EMDR, Schema Therapy, Mindfulness (MBCT), Dialectical Behaviour Therapy (DBT), ACT or Neuroaffective-CBT (NA-CBT). With a career spanning over almost 30 years in the mental health field, I dotted many 'i's and crossed many 't's in mental health, forensic and substance misuse clinics as a clinical head, clinical supervisor or just psychotherapist. I have always felt drawn towards complex cases of low self-esteem and childhood trauma but also at the other end of the spectrum by the prospect of enhancing performance and abilities through a positive mindset. As such, over the years I developed my own behavioural approaches.. Neuroaffective-CBT mostly for chronic negative emotions, Emotion-Focused CBT for interpersonal/relationship problems and Sports-CBT for enhancing performance in sports.
