Strong communication between all parties is vital. ‼️⁣

It baffles me when consultants and physios don’t have a clear channel of communication when working together delivering patient care. 🫤 ⁣

The patients ends up being used as a proxy messenger, relaying messages between. 💬⁣

I 𝐀𝐋𝗪𝐀𝐘𝐒 reach out to a consultant or other clinician when we are mutually seeing a patient if there isn’t already a channel of communication there. It undoubtedly makes giving best care easier. 😊 ⁣

Conservative Osteoarthritis Management!⁣

It’s clear from all the green which ones we should all be sticking to when it comes to non-surgical (conservative) management. 😃 ⁣

This table summarises guidance made by four parties; ACR (American College of Rheumatology), EULAR (European League Against Rheumatism), AAOS (American Academy of Orthopaedic Surgeons) and OARSI (Osteoarthritis Research Society International). ⁣

Seen this a few times recently. It really makes no sense. 🤷🏼‍♂️ ⁣

So there’s no confusion - in the 1960s the British Ministry of Transport introduced a yearly test for cars on the road to ensure they were safe. 🇬🇧 🚗 ⁣

Therefore this became shortened to an ‘M.O.T’ which has since become a sort of synonym for regular check up. Interestingly the Ministry of Transport no longer exists and is now the Department for Transport. I supposed DfT doesn’t roll of the tongue so well. ⁣

Anywho, it is now relatively common for physios and other musculoskeletal clinicians to refer to semi-regular or ‘maintenance sessions’ as an M.O.T 😬 ⁣

Your body is strong, robust, adaptable and able to heal itself. All very different from my 2007 Ford Fiesta S. 😆 ⁣

If people want to spend their income on a massage or another therapy then that is absolutely fine. It can make people feel great and if they feel better and this opens the door to training better then I have no problem with them allocating resources to that. 👌🏼⁣

If it is an athlete or keen exerciser and you’re testing metrics such as strength, mobility, stability and endurance semi-regularly to hone in their training plans then surely we can use a better word than the initialism of Ministry of Transport. ⁉️⁉️⁉️⁣

We all know weight loss is one of the core pillars in osteoarthritis management. ⁣

The updated NICE guidelines published on 19/10/22 outline the above guidance for clinicians seeing OA patients who are overweight or obese. ⁣

Key things to know:⁣

🔹 The available evidence is heavily based on those with knee OA. 📚 ⁣

🔹 Achieving successful weight loss is challenging and NICE recommended that appropriate support should be provided. ⁣

🔹 Providing a target (10%) should be used to help motivate people. 🎯 ⁣

Imagine everyone is walking around with a floating ‘To Do’ list above their head. 📃⁣

As musculoskeletal clinicians, it is within our scope to potentially help tick off one of those issues. We probably can’t help with their mortgage but hopefully we can with their low back pain! 😃 ⁣

I am always talking about this idea of a floating ‘To Do’ list. How can we get it so our patients and clients feel this problem is fixed or they have high self-efficacy to manage themselves going forward? 🤔 ⁣

That’s the goal!! ⁣

Our outcome should NEVER be to develop this dependency on us. It’s frankly criminal. ⁣

Acute OA Flare-ups: Using red counters and blue counters! 🧮⁣

When a person with a joint (such as; hip, knee, ankle) that is known to have some degree of OA has a sudden worsening in symptoms, for instance, after an awkward movement, we can look at this as a sudden increase in red counters. 📈⁣

We can help calm things back down by removing red counters and introducing blue counters. 📉⁣

This model is not always applicable but for the right patient, can be really helpful! 😄⁣

When helping our patients within our scope as Physios/Osteos/Chiros/Sports Therapists we know this. We know patients will be lucky to get better week on week on week. 🗓 🚫⁣⁣

Progress will fluctuate, niggles will happen, we may overdo things. ⁣⁣
⁣⁣
This is where we need to communicate clearly with our patients. Be there for them during these setbacks and help them come out the other side!!! 😄⁣

Surgery rehab has to be adjustable- as easily as the gas pedal on our cars. ⁣

We can push HARD at times and when things slow and we ease off, be re-assured you’re not automatically in reverse. 😄⁣

Vit. C after orthopaedic surgery⁉️🍊⁣

Vitamin C is a vital antioxidant. Crucial for human health. 🥝 ⁣

In the field of orthopaedics there is moderate evidence that Vit. C can aid functional outcomes, reduce post-operative pain and decrease risk of CRPS (Chronic Regional Pain Syndrome). 🏥 ⁣

Maybe we shouldn’t be routinely recommending it just yet but encouraging your patients to eat a few satsuma’s or kiwi’s probably isn’t a bad idea! 😄⁣

Here’s a few similarities I draw between my experience preparing for a competition and helping patients/clients with their issue as a physio:⁣

(Before we start - what an odd “sport” bodybuilding/physique is!!) 😆 ⁣

1) Nothing is linear - don’t celebrate too much when things go well and don’t beat yourself up during the dips. Make sure the correct trajectory is maintained. 💹⁣

2) Having someone in your corner is invaluable - in competition prep it was training partner(s), in injury rehab it is our role as a physio/clinician to be in our patients corner! 🥊 ⁣

3) Everything goes smoother when you maintain a healthy diet!! 😂 😆 ⁣
