Community Pulmonary Rehab Team Gain Prestigious Accreditation

We’re proud to announce that our Community Pulmonary Rehabilitation Team has achieved full accreditation from the Royal College of Physicians — becoming one of only 34 fully accredited services in England and Wales, and the third in Cheshire & Merseyside ICB.

This milestone reflects three years of dedicated work meeting 40 standards aligned with the British Thoracic Society’s Quality Standards for Pulmonary Rehabilitation.
The team demonstrated excellence across:
✅ Leadership & strategy
✅ Staffing
✅ Person-centred care
✅ Clinical effectiveness
✅ Risk & safety
✅ Improvement & innovation

Their work on addressing health inequalities was also recognised nationally. Sarah and Callum were recently invited to speak at the Royal College of Physicians’ Accreditation Unit Network Day, sharing how a QI project using the HEA Tool helped tackle barriers to PR referral and attendance in Winsford. Their learning has now been embedded across the service, improving uptake and reducing Did Not Attends (DNAs).

This is an outstanding achievement, made possible by the team’s commitment, innovation and passion for improving care for our local population.

CCICP Community Diabetes Team are awarded a GOLD Quality Matters Accreditation 🏆

During November CCICP Quality Matters team undertook a Quality Matters accreditation visit to our Community Diabetes Service.

The team demonstrated kind compassionate consultations with patients who accessed the service. The service demonstrated that they supported the wider population through diabetes groups within the community and supported training and development of staff and partners within and outside of the NHS Trust.
The team portrayed a culture of continuous improvement and evidenced that approach was integral to service delivery and improvement.
The services embraces those with health inequalities and could demonstrate how reasonable adjustments were put in place to support patients when needed.

Occupational Therapy Apprentices Reduce Number of Falls by 80% at Elmhurst Intermediate Care Centre

A year ago, two Occupational Therapy Apprentices were challenged to reduce falls within Elmhurst by 30%. Now 12 months on, they have reduced them by a fantastic 80% and increased falls prevention awareness by 67%.

Three activities were introduced for patients to participate in:
1. Breakfast Club
2. Activities of Daily Living Circuit Training
3. Seated Movement Bingo

Throughout all of the activities, education on falls prevention was delivered. Patient feedback has been incredibly positive and reflected in the high engagement.
