Churchfields Homecare are looking to recruit an Assistant Care Co-Ordinator. This is an exciting opportunity for the right candidate.
Key Responsibilities:
Manage a two-weekly rota system, prioritising the needs of individuals we support to ensure consistency and continuity of care.
Oversee the daily operations of the rota, ensuring all individuals receive their planned care calls as scheduled.
Build and maintain strong working relationships with individuals, their families, and other stakeholders, ensuring that care and support needs are met with a person-centred approach.
Communicate effectively with care staff to ensure a collaborative and supportive working environment, helping to ensure the team feels valued.
Respond to new care package requests from both local authorities and private clients.
Liaise with families, social workers, GPs, district nurses, occupational therapists, and other professionals to ensure seamless, multi-agency coordination of care.
Regularly monitor and adjust care staff rotas to optimise travel time and minimise unnecessary driving.
Arrange and conduct practice-based spot checks for care staff to ensure adherence to high standards of care.
Address and manage low-level complaints in line with company policies and procedures.
Perform various administrative duties, including email correspondence, generating rota reports, attending meetings as required, and maintaining document organisation in compliance with GDPR.
Desirable Skills and Experience:
Minimum of QCF Level 2 qualification in Health and Social Care.
At least one year of experience in rota/shift management within the care sector.
Experience in an office-based role, with proficiency in administrative tasks.
Knowledge of risk assessing and risk management processes.
Excellent communication skills, both written and verbal, and the ability to build positive relationships with individuals and staff.
Strong organisational skills and flexibility to adapt to varying daily tasks and schedules.
Competence in using Microsoft Office applications (Word, Excel) and general IT systems.
A commitment to ensuring individuals receive regular carers and minimal changes to their care plans.
Additional Requirements:
Due to the nature of the role, there may be occasions where you will need to provide hands-on care in the field when there is a staffing shortage.
On-call duties are required on a rota basis, with one weekend in four and occasional weekday coverage. On-call duties are compensated in addition to regular pay.
Working Hours:
Monday and Thursday: 08:30 – 15:30
Friday: 08:30 – 15:00
Additional on-call duties as required.
Salary:
Starting at £12.30 per hour.
