Diabetic Charcot foot is a devastating condition. The combination of neuropathy and pathological inflammation leads to collapse with bone destruction. Usually patients present with a hot swollen foot that is painless.

This case progressed over a 12 month period despite offloading and casting.

Bony collapse leaves the foot unstable and vulnerable to ulceration.

Reconstructive surgery is particularly challenging. A combination of midfoot excision, beaming and subtalar stabilisation with bone grafting was used to realign the osseous structures.
