Tailored Transitions Ltd Tailored Transitions Ltd was founded in December 2015, by two passionate learning disability Nurses.

Tailored Transitions Ltd supports young adults with complex medical, physical and learning needs in a bespoke small scale residential basis, to live their best life. With three small homes based in Ferndown, New milton and Bearcross.

What a fantastic afternoon, we are so proud to be celebrating 5 years of Oakside, where has the time gone!

It was so lovely to see so many of you join us today and the weather was perfect too, I think the photos say it all.

We would be lying if we said the last 5 years were a breeze especially with Covid in there, but supporting our young adults to live their best lives is what we love to do.

Here's to our next celebration being 10 years of Tailored Transitions in 2027.
