We are two of the Island's GPs who feel that currently the NHS services do not give patients enough time to have a meaningful discussion.We know this is not the fault of the local GPs, but we decided to offer an alternative way of consulting a doctor. This means You can decide how much time you want to spend with us and what topics you would like to discuss. We don't have any government targets to

meet so we can fully focus on what matters to you. We are currently going to work at Manx Medical Imaging at Village Walk, Onchan, before moving to out own premises.

“I’M JUST TIRED”

Most men don’t book an appointment because of hormones.
They book because they’re tired.

From my desk, what I hear is:
“I’m just exhausted”
“I’ve lost my edge a bit”
“I don’t feel like myself anymore”
And often, it gets put down to work, stress, or getting older.

Sometimes that’s true.
But sometimes, there is an underlying hormonal shift, particularly testosterone, that is worth looking into.

At Mannin Doctors we’re now seeing more men in their 40s and beyond who recognise something has changed, but aren’t quite sure what.
This isn’t about jumping to treatment.
It’s about understanding what’s going on first.
Because “just tired” isn’t a diagnosis.

From my desk this week....
A patient came to see me recently because something didn’t quite sit right.

She had been prescribed a weight loss injection elsewhere, and although the consultation had seemed straightforward, she wasn’t entirely comfortable starting it.
So we talked.
Not just about her weight, but about her general health, her background, and her life.
What we call taking a proper history.

During that conversation, two significant issues came to light.

One was a set of symptoms strongly suggestive of a tumour that required urgent investigation.
The other was a situation in her personal life that also needed addressing.

Either one of those would have been a reason not to start weight loss medication.

Within six weeks, she had been diagnosed, treated, and was feeling remarkably well again.
Interestingly, although her weight hasn’t changed significantly, she has no intention of using injections at this stage.
Because the real issue was never her weight.

She has kindly allowed me to share this, as it highlights something very important.
Medication should never be prescribed in isolation.

It should always take into account:
your full medical history
your current symptoms
your wider life circumstances
and anything else you may be taking
If that hasn’t happened, I would always suggest pausing.

Go back and ask questions.
Or seek a second opinion before starting treatment.

With the amount of information now available online, and the increasing access to non-medical prescribing, this is becoming more important than ever.

From my desk this week - We continue to be very busy at Mannin Doctors, and one thing I’m seeing more and more is confusion around blood test results.

Patients are either told everything is “normal”, or they don’t chase the results at all and quite understandably assume that no news is good news.

Unfortunately, that isn’t always the case.

I recently looked after a lady who had been told over several years that her blood tests were fine. She had previously been noted to be pre-diabetic, and her tests were repeated annually.

When we looked more closely, her diabetic markers had not actually been checked.

She became quite unwell and needed urgent treatment to bring her diabetes under control.

She has kindly given permission for me to share her story, because both of us feel this is exactly the sort of situation where nobody has done anything wrong, but the system and communication haven’t quite aligned.

So my advice is simple, and important.

If you have blood tests done:

Make sure the right tests have been requested.
Make sure you actually receive the results.
And if you don’t understand them, ask.

It’s also worth keeping an eye on your core markers as you get older, particularly if you have a family history or previous concerns:

blood count
kidney function
liver function
cholesterol
and your diabetes score

I have also seen situations where results have simply not made it from the clinic to the lab or back again, and the assumption has been that everything is fine.

So please don’t assume.

If you have any doubts, speak to your practice team.

And if you’d like someone to sit down and go through things properly with you, we will always make the time.

Because there is very little value in having tests done if you don’t get clear answers at the end of it.

what drains you every week
what symptom have you been ignoring
what do you keep pushing to next month
what would improve your life if it felt easier

Then pick one next step for this week. One.

And if you feel stuck, or your symptoms are not improving, don’t keep trying to self manage forever. That’s what we’re here for.

People are not struggling to explain symptoms because they are vague, they are struggling because they have not had the time or space to talk it through properly.

Start with this framework, you can even write it in your notes before you come in.

1) What is the main problem, in one sentence
No backstory first, just the headline.

2) When did it start, and what has changed
Sudden or gradual, getting worse, or staying the same.

3) What does it feel like
Sharp, tight, heavy, burning, throbbing, dizzy, breathless.

4) What makes it better, or worse
Movement, food, rest, stress, time of day.

5) What else is happening
Fever, bleeding, swelling, weight loss, sleep disruption, mood changes.

6) What are you worried it might be
Say it out loud. It helps us address the fear, and the facts, together.

You deserve a clinician who takes you seriously, and you deserve time to talk.

Please don’t wait until you hit crisis point.
The earlier we look at something, the easier it is to manage.

Book a proper review if:
you are exhausted all the time and rest isn’t helping
your sleep is broken most nights
your mood has changed and it’s lingering
you are getting breathless doing normal tasks
you have persistent pain or symptoms that are not settling
you keep saying “I’ll sort it after this busy patch” and the patch never ends

You do not need to be at breaking point to deserve support.
