Guardian Response Ltd Esteem Care Dumfries and Galloway Ltd are providers of Domiciliary Care & Emergency Response Service We employ approximately 30 staff in Dumfries and Annan.

INTRODUCTION

Guardian Response Ltd was established in January 2003 to enable vulnerable people to remain independent in their own home. It is a small family run business owned by Joy and John Donowho and their daughter Corinne Martin who is also the Company Operations Director The agency operates 365 days a year and the phones are manned 24 hours per day. To ensure we employ reliable, caring staff we have a very robust recruitment procedure in place


OUR MISSION

At Guardian Response we aim to be recognised as a unique service to the public offering Emergency Responders to the care call system and as providers of high quality home care. Our emphasis is on delivering a professional quality service which meets and exceeds the service user’s needs


AIMS & OBJECTIVES

Our aim is to be the No1 Provider in Dumfries and Galloway and to provide the highest level of care and assistance appropriate to the needs of our service users. We aim to promote independence and participation at all levels in our service by using person centred care planning and assessment, resulting in a holistic approach to our service users care needs. We like to be as unobtrusive as possible within the service user’s home especially where the home is shared with a partner. We extend our respect to service user’s relatives, carers or other advocates whilst continuing to embrace service user confidentiality. We have the same philosophy towards our service users whatever the frequency of our services, for example, daily, weekly or to help with recuperation or holiday relief cover for other carers (or even assistance to holidaymakers). We aim to encourage individual choice and recognise different social lifestyles and home environments. The service we provide is client-focussed. We aim to meet the requirements of each individual’s assessment and to monitor its progress so as to meet changing needs. Our Senior Carer/Manager will visit and discuss any requirements, preferences and advise accordingly. A care plan will be designed to meet the specific needs with service users input and that of their relatives and carers. Carers will then be assigned to carry out specific requirements. At the end of the first month, after the service has commenced, the Senior Carer/Manager will contact the client and carry out a small review to make sure that the service user is satisfied with the care he/she is receiving. On going reviews and questionnaires are completed to give feedback on the service.

Esteem is a Care provider proving Care and Support services in Stranraer. The post is for 37.5 hours a week with oncall on a rolling rota pattern evening and weekends. Rate of pay £14.90 ph for office hours

We are looking to recruit an Care Coordinator to work along side our Team Leader and Service Manager.

As an Care Coordinator your key responsibilities will be

1. PURPOSE OF JOB

1.1 To co-ordinate the delivery of care services to all Esteem Care Ltd service users.

1.2 Establish a positive relationship with staff, management, care workers and clients and their families

1.3 To liaise with all relevant health and social care parties concerning service user and service user issues.

1.4 To maintain the integrity and accuracy of all data stored on our computer system, ensuring all bookings and paperwork are correctly recorded and information is kept up to date at all times.

1.5 Checking of timesheets/mileage sheets of carers weekly, and prepare monthly wage report

1.6 Preparation of staff rotas, arranging cover and shift changes

1.7 To reconcile CM2000 call monitoring on a daily basis when the other Coordinator is absent

1.8 To be responsible for training of staff, and keeping training records up to date.

1.9 Communicate frequently with Service manager on any issues etc

1.10 Attend any relevant meetings with local authorities, social services etc

1.11 To have knowledge of Adult Support and Protection procedures, and also Self Directed Support

1.12 To play a lead role in the development of good community care practices and procedures in relation to Esteem Care Ltd

1.13 To work in conjunction with colleagues to assist in the monitoring of all care delivery practice and procedures.

1.14 To provide a cover care service and deliver care as may be required from time to time when emergencies or short term requirements present such a need and in the absence of any other member of staff who would otherwise be able to assist, and also cover the Emergency Responding service when required.

1.15 To record all communications into and out of the office in the prescribed manner, using the correct documentation.

1.16 To conduct and participate in unannounced spot-check supervisory and audit visits at service users homes, to record the outcomes of these meetings on the appropriate forms and return and file in the office.

1.17 To participate in work shadowing and the integration of care/support workers into their new working environment

1.18 To liaise with management in supervising and assessing new members of staff during their initial induction and shadowing period.

1.19 To participate in quality assurance reviews and other audit and control initiatives as directed by management

1.20 To undertake any other reasonable duties required to ensure the efficient and effective delivery of care services of the operation of the organisation, as requested by the Service Manager

Person Specification

Experience: Relevant experience in a related field or personal experience.

Education, Qualifications and Training: SVQ level 3 in care, and willing to develop IT and administration skills, within a specified time.

Skills, Abilities and Knowledge: Good organisation skills. The ability to work on your own initiative and manage your time effectively. The ability to manage pressure and conflicting demands. The ability to prioritise tasks to ensure the effective management of the systems involved in the day to day running of the business. Strong oral and written communication skills. Ability to reflect on practice.

Interpersonal and Social Skills: Good verbal and non-verbal communication skills. Observe standards of dress code appropriate to the post. Tact, discretion and respect for confidentiality. To be honest, reliable and sensitive to the needs of others.
