EZZE Healthcare (EHC) is looking for carers in the community to assist people that require support with household duties and personal care, and to enable them to maintain their independence whilst living at home.
Clients can include elderly, people with ill heath and young adults 18+, learning disabilities, mental health problems, sensory impairment, and people with dementia, complex needs or physical disabilities.
A drivers license and access to a vehicle is a requirement.
Experience in health and social care is required.
Key Responsibilities:
-To ensure that customers have choice and control over the service/s they receive and the outcomes they wish to achieve.
-To show commitment to the mental and physical wellbeing of our customers by supporting them within a variety of settings,
-To meet outcome-based performance targets to support our customers’ independence and self-sufficiency skills throughout their care plan
-Use of EHC’s staff and customer information system , ensuring attention to detail in the reporting and recording of care plans
-Develop and maintain strong relationships and communication links with our customers, colleagues and other professionals, e.g. GP’s, District Nurses, Social Workers to ensure a comprehensive and versatile care provision
-Ensure EHC's value of 'Person Centred' Care is offered to our Service Users at all times.
