Some time since my last post - but lots of interesting and challenging problems always come to me.

In the Ebchester clinic, a female pt with an 18 month problem with her right knee and outer to front of shin, following a knee pain that came on for no reason. Her knee was injected - primarily for OA - but her burning pain in the shin area did not change, and she was in pain and couldn't walk much at all. This was missed by various therapists and not even assessed fully.
From detailed assessment she had a superior Tibio-Fibular stiffness with traction of the peroneal nerve at this point leading to her nerve/neural pain. Now after her 4th session she is totally pain free and very happy.

I've had a few bad - but different - back pains recently.

1 - 55 yr old male - really nasty discal pain leading to huge neural pain and reduced function. Initially, possibly needing surgical intervention, BUT once MRI carried out it was agreed to follow a conservative approach. 2 months later 75-80% better.Much reduced pain, muscle power slowly returning and altered sensation in feet reducing and returning to normal. He is now beginning to do more flexibility and strength exercises.

2 - Female aged 36 with nasty pain and a lateral shift - pelvis to one side - which is a common sign of back - and discal - pain. Also reduced muscle power and function. Not happy and unable to work. 4 sessions later within 3 weeks, she is back at work PT to FT, has no shift and hardly any pain until the end of the day and fatigue sets in. Now following a specific exercise programme.

3 -54 year old male, farmer. Back pain of 3-4 weeks duration - good and bad - but suddenly worse pain. No shift but pain with cough and sneeze plus some altered sensation in right leg/foot. 7 weeks later back at work fully though not lifting heavy equipment regularly as yet. Minor intermittent soreness and leg feeling virtually normal. Gradually exercising more and feeling happy.

All back presentations are different and require good detailed movement assessment.

Other interesting presentations include -
1 - Female aged 40 - runner with Patello-Femoral problems
2 - Female aged 65 with chronic OA neck pain and severe restricted movements
3- Male aged 55 - long term mid spinal (Thoracic) into the upper lumbar region pain happening only when out running after 3km!

Happily all are improving with a combination of detailed movement assessment, Deep Oscillation Therapy, Acupuncture both IMS and Traditional Chinese Medicine points, manual therapy, taping and more.

Integrated therapy and good specific exercise structure is essential for successful management with all pains and injuries.

An update on some recent clinical work within Chris Boynes Physio.
Had a long standing hip/groin patient - sportsman. Imaged with US and MRI re possible problems. Some tendinopathy highlighted but all else good, yet pain remained. Full pain reduction and increased function with a combination of connective tissue, joint mobility, specific exercise rehabilitation and the subtle Dermoneuromodulation (DNM) work on part of his pelvic floor.
Long standing 'Frozen shoulder' management.
Successful rehabilitation of a borderline Type 3/5 Acromio-Clavicular disruption. Thus avoiding surgery.
On-going assessing and managing various detailed presentations of both spine and hip.
Once again a mixture of a detailed movement assessment, testing muscle function and strength, pain and its core cause(s), and then treating with joint and soft tissue, specific structured exercise rehabilitation programmes and Deep Oscillation Therapy enabling patients/athletes to return to full function and sport.

Been a very busy last week in Ebchester and Fakenham.
Once again a highly interesting and challenging variety of folks.
Chronic spinal pain.
Post op hips and knees.
A chronic adductor problem in young footballer - now resolving with manual therapy and specific exercise rehab.
Using DermoNeuroModulation with necks and shoulder problems,and long term hip/ lower limb presentations.
Now en route to Penang - then Kuala Lumpur and Singapore - then home.
No clinics now until 3rd and 6th Feb onwards.
And the clinics are virtually full.

Hello again.
Just a small update on more interesting cases I'm dealing with.
A different left sided superior Tibio-Fibular injury suggestive of boney degeneration from the actual Tib-Fem knee joint, and / or a neural affect with the peroneal nerve.
A very resistant 5 month old extremely stiff Total Knee Replacement needing detailed manual therapy.
Visiting a gent with MS and utilising muscle stimulation for him, something that lots of other patients could use who require good muscle activation work, especially post-operatively.
A female pt aged 37, with long standing lower back and right sided SIJ problems, doing fantastically well with a very good SIJ/pelvic brace, which allows and increases better muscle activation and strengthening exercises at the trunk and lower limbs.
Post dislocated right shoulder 6 weeks into rehab doing very well.

Hi Folks.
Another interesting week managing acute knees to post op THR, to lingering right sided spinal pain which is not changing with other manual therapies.
Plus some lateral elbow pain and a headache presentation. And monitoring a RTP (Return to play)soccer player post ankle injury.
Most - if not all problems - are treatable, and changeable, with some interaction of manual therapy, Acupuncture - both traditional and Intramuscular Stimulation (IMS), exercise therapy and undoubtedly Deep Oscillation Therapy.

Morning everyone from a cloudier but still warmish Ebchester, County Durham.
Another very busy week in Fakenham seeing a wide variety of pain and movement problems. From knee and hip replacements to runners knees and weight lifters back/hip pain. And cervical spine acute discal pain.
All managed with a mixture of detailed assessment and diagnoses, then Deep Oscillation Therapy, Acupuncture both Dry Needling and Traditional Chinese Points. Plus exercise, taping and functional advice.
However, I would ask you to please look at the video on our updated website regarding Deep Oscillation Therapy. This unique treatment helps with (almost) everything and is an integral part of all my treatments. Below is a lovely testimonial on the huge beneficial changes it can make.

I have an autoimmune inflammatory disease which affects my joints and soft tissues. Just had another successful video consultation. One happy patient. She now has a diagnosis for her problem, and some exercises, and advice to follow.
I’m not saying this replaces face to face and hands on consultations, but it will save time when we do meet in clinic, and it has reduced the patients anxiety in the meantime.
