Hart Health Partnership GP Noticeboard

Hart Health Partnership GP Noticeboard Hart Health Partnership is the merged Fleet GP practices of Fleet Medical Centre & Branksomewood Road

"It is with excitement that Fleet Medical Centre and Branksomewood Healthcare Centre proudly announce that they will be merging as of 1st April 2023. It is a big piece of work to merge our 2 practices and we are keen to do this to protect and preserve the provision of healthcare services in this part of Fleet by creating stability, recruitment and training options for the future. We already work closely together as member practices of Fleet Primary Care Network and we have good relationships with the other 2 local surgeries -Richmond Surgery and Crondall New Surgery- who are supportive of our merger."

Another article with resources for people with chronic pain or any condition that brings fatigue. We know many of you struggle with managing this aspect. This article is about pacing—it can be hard to do but brings the benefit of feeling more in control of your condition. It includes a downloadable, editable chart to help. We’ll be creating our own HHP version soon: The power of pacing: Managing energy with a chronic illness. What other tips or resources would be helpful for your or a loved one’s condition?

Getting things done with a rare disease or chronic illness can be difficult at best. Have you tried pacing?

It's PoTS Awareness Month! At HHP, we're seeing increasing numbers of patients with symptoms of postural orthostatic Tachycardia syndrome—sometimes as part of Long Covid. This article on Special Needs Jungle by Tania, who's also an HHP PPG member, explains the condition and offers useful resources: https://www.specialneedsjungle.com/living-pots-understanding-postural-orthostatic-tachycardia-syndrome-schools-help/

🎀 October is Breast Cancer Awareness Month - a time to learn, support, and advocate for everyone affected 💖

Breast cancer doesn’t just affect women. Men can get it too. It can also impact transgender people, who may face extra barriers to awareness, diagnosis, and care.

Here’s what to keep in mind:
🩺 Know the signs in everyone - lumps, changes in shape, ni**le changes, skin dimpling
🔍 Men, don’t ignore changes in your chest or ni**le - although less common, breast cancer can still occur in men
🏳️‍⚧️ Trans and nonbinary people: your risks and body may change with hormone therapy or surgeries - always feel empowered to ask for care and checkups

Fleet PCN is composed of Hart Health Partnership, Richmond Surgery and Crondall New Surgery. We work very well together and are all very dedicated to keeping our community healthy. 💪

🌟What is a Primary Care Network (PCN)? 🏥💙

🔍Understanding PCNs:
A Primary Care Network is a group of local GP practices that work collaboratively to provide comprehensive and integrated care to our community.

💼🤝These networks aim to deliver more personalised, accessible, and efficient healthcare services for you and your loved ones.

🤝 Emphasising Teamwork:
At the heart of PCNs are healthcare professionals like GPs, nurses, pharmacists, social workers, and other allied health professionals. Together, they form a dynamic team focused on addressing your health needs holistically. 🤗💡

What Our PCN Offers:
Our PCN is a prime example of a collaborative healthcare network. We bring together local GP practices with a shared vision to improve patient care and enhance the health and well-being of our community. 🏥🌟

🏋️‍♂️A Stronger Together Approach:
By pooling resources and expertise, PCNs can offer a wider range of services, making healthcare more accessible and tailored to your unique needs. 💪🔗
