Health Connectors are your GP practice Social Prescribing team in Mendip, Somerset.

Working with a Health Connector means taking time together to understand the complexities of your life, including the challenges you face, your strengths, and the support you may already have from friends, family, or your community. We help you figure out what matters most and what steps you want to take next.

We offer our patients who are living with pain the opportunity to take part in a programme called Live Well with Pain.

Our next 6 week Live Well With Pain Group starts on Thursday 16th April. Sessions will take place every Thursday at the practice from 11am to 1pm.

This programme is a face to face group which provides you with a step by step guide to living well despite your persistent pain. When people self-manage their pain, their quality of life improves. Eventually it stops dominating their day and they begin to get more out of life.

Knowing how to self-manage your pain is not automatic, but it is something that you can learn. Our Live Well With Pain Group can help you with this.

Come and see our digital health team this week if you need help with things like downloading and using the NHS App, using the text links we send out to book appointments, ordering repeat prescriptions online, setting up a linked profile (proxy access) so you can use the NHS App on behalf of a relative or friend and so on.

Here's where you can find the team this week (no appointments needed but do bring your mobile phone or tablet along with you if you can).

Unfortunately, our digital health pop up at the practice last weekend couldn't go ahead but we have rescheduled it for tomorrow - Saturday 14th March from 8:45am to 12:45pm.

Come along and see the team for help, including:

✔ Setting up and using the NHS App
✔ Ordering repeat prescriptions on the NHS App - quick and easy to do - let us show you how!
✔ Advice on creating a linked profile (also known as proxy access) for a child or other adult you care for (if both registered with us)
✔ Using our online forms for things like updating contact details, requesting fit notes etc

Come to a Menopause Discussion Group at Frome Medical Practice

Our Menopause Discussion Group is for patients registered with us who are considering taking HRT for the first time.

📅 Tuesday 28th April 2026
🕒 6.15pm to 8.15pm (you will need to be able to attend for the whole session)
📍 Frome Medical Practice

During the session we will talk about:

Menopause symptoms and how they vary from person to person.
Ways to manage symptoms.
HRT treatment options, including benefits and risks.
Alternative treatments, such as lifestyle changes.
Signposting to other support resources

Past, Present and Future - In One Immersive Pop-up

For five days only, at the bottom of Cheap Street, a pop-up comes alive with five years of Green and Healthy Frome (GHF). From Tuesday 24 to Saturday 28 March, drop into the GHF Studio and Imaginarium to put your mark on the map, explore, photographs, artefacts & beautifully illustrated archives, by local artist Jenna Herman - tracing the roots of sustainability in our town - while green-dreaming Frome's future.

Peer into the Imaginarium, meet Frome Town Council's Resilience team, join workshops throughout the week - including Frome Families for the Future (Friday) and Mending Together with Everyone Needs Pockets (Saturday, 10am–2pm). Plus: pick up your free copy of the brand new Frome Green Directory!

This pop-up marks the end of the National Lottery funding for the Green and Healthy Frome partnership between Frome Medical Practice, Frome Town Council and Edventure - five years of collaboration, experimentation and community building. But it also asks the bigger question: what steps does Frome need to take next, to address the climate crisis?

Stay safe - stockpiling medicine can be dangerous to children and pets.

We've 2 new start dates for our Habits for Health programme to let you know about.

Habits for Health is a 6 week group that supports you to focus on what matters most for your health, making small changes that can lead to a healthier life. Whether it’s moving more, sleeping better or finding balance in day to day life, starting with small steps can make a big difference. It's run by our health connections team and we've 2 groups starting in April.

Meningitis - know the symptoms and what to do if you spot them

Meningitis is an infection of the protective membranes that surround the brain and spinal cord (meninges). It can affect anyone, but is most common in babies, young children, teenagers and young adults.

Meningitis can be very serious if not treated quickly. It can cause life-threatening sepsis and result in permanent damage to the brain or nerves.

Symptoms of meningitis develop suddenly and can include:

a high temperature (fever)
being sick
a headache
a rash that does not fade when a glass is rolled over it (but a rash will not always develop)
a stiff neck
a dislike of bright lights
drowsiness or unresponsiveness
seizures (fits)

These symptoms can appear in any order. You do not always get all the symptoms.

When to get medical help - call 999 for an ambulance or go to your nearest A&E immediately if you think you or someone you look after could have meningitis or sepsis.

Join us for our first digital confidence session, taking place this week.

The topic of staying safe online and avoiding scams is something many people have spoken to us about at our digital drop in sessions. We're delighted that the Avon and Somerset Police Cyber Crime Unit will be joining us for this session.

We are taking part in the Frome Kindness Festival this week!

Members of the practice Patient Participation Group and Health Connections Team will be chatting to patients visiting the practice on 17th, 18th and 19th March to find out about their experience of kindness at the practice and what more kindness could look like.

The suggestions that patients share will help shape a more compassionate and connected practice for everyone.

You can meet the Kindness Crew at the practice and have a chat at the following times:

Tuesday 17th March, 10am to 2pm
Wednesday 18th March, 10am to 2pm
Thursday 19th March, 10am to 2pm

As a practice, we aim to be a kind organisation where we care for each other, as well as for our patients, and where we also care for the world around us in responsible and sustainable ways. The kindness festival is an opportunity for us to encourage conversations around what a kind organisation looks like and we are looking forward to speaking to our patients about this over the course of the festival this year.
