Here at the community diabetes team we firmly believe that knowledge is power. Evidence shows that people with diabetes who know how to manage their condition have better long term outcomes. It is recommended that everyone diagnosed with type 2 diabetes attends an education session so that they can learn about their condition and how to manage it. We offer sessions face to face across the county, online and evening sessions too. Use our online referral page to access our friendly information sessions. https://www.ghc.nhs.uk/our-teams-and-services/diabetes/diabetes-education-sessions-referral-form/
