If you have an outpatient appointment at our hospital, and need to get in touch with our team, there is now a new way that you can do this via a form on our website.
We recognise that it can be difficult sometimes to get through via phone. Our team receive a high volume of calls and, due to limitations with the existing telephone system, callers are not able to join a queue.
We have been looking at how we can improve the ways you can get in touch with us and the online form is a first step, to give another method to get in touch with our Outpatient team.
To use the form you’ll need to have some information to hand including;

• Patient’s full name
• Patient’s date of birth
• Hospital number or NHS number (if known)
• Appointment details

The form can be found on our website here; https://www.jpaget.nhs.uk/patients-visitors/outpatient-clinics/

This is in addition to the option to reply to an appointment reminder text message. If you receive a text reminder and are unable to attend your appointment, please reply to the text message if you are able to do so.
Please also continue to use the number on your appointment letter if you do not have online or text message access.
