Did you know that many people can recover and regain independence in their own home after illness, injury or a stay in hospital?

Our Reablement at Home service supports people to rebuild strength, confidence and everyday skills in familiar surroundings. Working alongside individuals and their families, our dedicated teams focus on supporting people to do things for themselves, not doing things to them.

✅ Helping people stay independent for longer
✅ Free at the point of delivery

For many, Reablement at Home is a positive step towards feeling confident, and back in control of daily life

🏐💙 Wheelchair Volleyball at our Community Inpatient Unit 💙🏐

We loved seeing the energy on display at our Community Inpatient Unit, where patients recently took part in a wheelchair volleyball session.

Adapted sports like this support physical rehabilitation, confidence as well as wellbeing, while also bringing people together for shared fun and achievement.

Moments like these show how staying active and trying something new can make a real difference 💪✨

Then ⬅️ vs Now➡️ - Nursing Journeys

At Care Plus Group, every nurse brings a unique journey, but they all share the same dedication: caring for our community.💜

✨ Tracy - Senior Nurse (Cancer Support)
"40 years ago, on the 5th May 1986, I walked into Grimsby School of Nursing with 19 other student nurses to commence my nursing career. After 3 years, I passed my finals and was lucky enough to get a job on an acute medical ward – jobs were hard to come by… I was one of 5 nurses to get a job.

I absolutely loved my 7 years working on C1, but wanted a new challenge and adventure, so I applied for a community nurse job – it took me a few attempts, but I finally got a job as an E-grade nurse. As much as I loved working at the hospital, community nursing was something else. To be welcomed into a person’s home is such a privilege and honour. Some of the patients almost became family as we spent so much time with them. I loved it so much that I stayed for almost 20 years.

Palliative care was always a priority for me, and I was lucky enough to secure a Band 6 post with the Haven team. I now get to look after patients at the end of life, providing personalised care and support to the most vulnerable at the most difficult time and provide support to their families and friends.

I have worked for community services and then for CPG for over 30 years, and I have loved every minute. I did retire 6 years ago – but couldn’t see myself not being a nurse, so I came back after 11 days and intend to work for a few more years yet."

✨ Georgina - Senior Operational Delivery Manager – Neighbourhood Health
Then vs Now: A Reflective Journey from Student Nurse to Operational Manager for Nursing.

My nursing journey has evolved over the years. It all began during my days as a student nurse, when I was learning the fundamentals of care and clinical skills. I’ve always been drawn to working with people, and initially believed radiography was my path. However, after some work experience in the radiology department, I quickly realised that my interest lay more in caring for people than in taking images. Watching the nurses in action, I knew instantly that nursing was what I wanted to do.

Joining Lincoln University as part of the first cohort for the degree-style nursing programme in 2005 was a pivotal moment for me. The training was unique and focused on community care and public health nursing, and it was then that I discovered my interest in community nursing. I thoroughly enjoyed my nurse training; everything was new, and I loved learning new skills and caring for patients.

Once qualified, I became a community nurse at the Welton Practice and joined a fantastic district nursing team. The mentorship and support there helped me develop my skills further, and I felt proud to care for my own patients. During this time, I discovered a real passion for Tissue Viability, which led me to my dream role as a tissue viability nurse at Care Plus Group in 2011. I spent twelve years in this role, enjoying a perfect balance between patient contact and developing my leadership abilities.

While my career has progressed into leadership and management roles, my heart remains with the patients and my drive to provide high-quality services and improve patient outcomes. As Operational Manager for Nursing, I now oversee services across multiple teams, balancing patient needs with workforce capacity and organisational priorities. Supporting teams and colleagues is something I truly enjoy, and I always strive to lead with compassion and care—the same values that drew me to nursing in the first place.

Throughout my journey, I’ve met some incredible people, patients, colleagues, and friends. I feel incredibly lucky to have had so many wonderful experiences along the way.

✨ Amy - Community Advanced Practitioner

Then
"My nursing journey began in 2011, inspired by coming from a family of nurses, including two generations of district nursing. From the start, community nursing felt like where I belonged. After qualifying in 2014, I secured my first role as a community nurse in Immingham, an incredible team then and remains incredible now. I was well supported through preceptorship, learning the foundations of community nursing and developing the skills and confidence I needed to grow.

As my experience developed, I completed the minor illness course, which strengthened my assessment skills and laid the groundwork for the advanced clinical decision-making I rely on daily. In 2016, I began the district nursing degree, a challenging but pivotal chapter in my career. Following a diagnosis of an adult learning difficulty, I received the support I needed to succeed, completing the course at master’s level and qualifying as a team leader."

Now
"Some of my fondest career memories come from my time as a team leader, supporting colleagues, managing complex patients, and developing both clinically and professionally. In 2020, I completed my V300 prescribing course, further expanding my ability to support patients with complex needs.

My passion for caring for complex patients led me to apply for a Community Advanced Practitioner role in 2021, which I was proud to secure. This role is incredibly rewarding. It is a privilege to support patients with long-term and complex health conditions to remain well at home and to help prevent unnecessary hospital admissions. I especially value that I continue to work closely with the same supportive team I started my career with, a reminder of how far I’ve come and the importance of strong teams in nursing."

✨ Jo - Registered Manager Reablement at Home
"I never wanted or thought I’d be a nurse. I actually started my career in art and illustration, but was made redundant in 2010 and started working at a local nursing home. What was ‘just a job’ soon became a career, and I graduated from the University of Lincoln in 2015. I started at Care Plus Group in the summer of 2015 with the Birkwood community nurses. Back then, there were just 4 of us responsible for that GP’s patients, and we worked closely in surgery. I then completed my district nursing, prescribing qualification and minor illness and went to work at the clinical assessment service, working up to a band 6 position. In 2019, I became the manager of the Haven team and absolutely loved looking after patients who were palliative. I am currently the registered manager of the Reablement at Home team, and I am very fortunate to work with such a hardworking, professional and caring bunch that makes my job a pleasure.

Nursing is a great career that can take you through so many different experiences, bad and good and whilst looking after others isn’t always easy, when patients get better or achieve their goals, it really does fill you with a sense of pride and achievement. Care Plus and the people who work here have been instrumental in my success, and I hope to continue this in whatever comes next for me in nursing."

✨ Julie - Advanced Urgent Care Practitioner
"Cor blimey, where do I start? I didn’t qualify as a nurse until I was 32, so later than some of our other very dedicated nurses. I initially started my journey on C1 Kendal, where I gained an invaluable medical background. From here, I was employed by what was then the Primary Care Trust (PCT), for those who remember where I joined as a community nurse. Over the years, I gained in-depth knowledge, confidence, and competence, where I really found my love for palliative care, although I never went down this route as a career. I continued with community nursing and undertook my DN training, which I completed in 2016… this makes me feel old.

I was lucky enough to also attend university to complete my PG Cert Practice Teacher and then onto my V300 prescribing qualification. I’ve been quite busy with my training; however, I must admit I love the prescribing element of my role. I left CPG for approx. 4 years, where I spent 2 years in GP land and also went on to work as an agency nurse across Lincolnshire in the various Urgent Care Centres. For the past year, I have been back at CPG working in the Community Urgent Care Team, which I love! I'm learning new skills, seeing and treating patients daily for various acute illnesses, and caring for palliative patients. I wouldn’t change my career for anything, truly love what I do."

✨ Debbie - Senior Nurse (Cancer support)
"I wanted to be a nurse since I was 4 years old (I am considerably older now!)
I started my nurse training in June 1983 at what was then the newly opened Grimsby District General Hospital (later named Diana Princess of Wales Hospital). In those wonderful days, nurses' hats, cloaks and the “Australian Lift” were the order of the day (things have changed somewhat!) I absolutely loved my training with the group I trained with.

I travelled to Leicester for my first post as there was generally a shortage of jobs for EN’s in 1985. My first post was in General Theatres (not a preference but a job! ), alas, I became allergic to Latex gloves, and that was my release! Thrown into the magical world of Medicine, I started on the wards as a D Grade. I completed my RGN Conversion Course in 1989 and then secured an E Grade post. I realised my interest in ward management, and with support from my Ward Sister, was successful in achieving an F Grade post. During these days, I gained amazing support from my peers, colleagues and tutors at Charles Frears School of Nursing in Leicester, who encouraged me to undertake further academic study. My greatest achievement to date is an MSC in Health Service Management.

I was successfully appointed as a Ward Manager / Sister in 1997 at Leicester General Hospital, where I remained for a couple of years.

In 2003, I saw an advert for a Ward Manager's post in DPOW on A1, namely the Oncology Haematology & Chemotherapy Unit, which I was successful in being appointed to in 2004. This was the most fantastic opportunity, to manage not just a ward but a unit, and a speciality at that.

Oncology Haematology was so new to me at this time, the ward team, Specialist Nurses and Consultants were so supportive, they all worked wonderfully as a team, and this would only improve as the years went on. In 2011, we moved into a refurbished location, The Amethyst Unit, which was wonderful and added so much more to the patient’s journey, in a very fitting environment, such a proud and rewarding time.

So after 18 years or so, I retired from that post. I worked as a staff nurse for 2 years at DPOW, but was then enticed to come and work for the wonderful Haven Team within CPG. A fabulous team led by Alison Wilson, who allowed me to completely change my career direction. Thanks also to the Senior Nurse team and carers for their ongoing support.
A massive move for me after 40 yrs in the acute setting, but a real eye opener to what the Community teams deal with daily. I work part-time with the Haven Team, and what a great bunch they are, providing care and compassion at the most difficult time in a person’s life and supporting their loved ones! Such a privilege to do this job & not ready to fully retire yet!"
