Diabetes Self-Management Programme (DSMP) - Type 2

This is a 7 week, 2 ½ hours per week self-management programme for anyone with Type 2 Diabetes (non-insulin dependent).

Some of the areas we cover are:

📝Monitoring and managing your Diabetes
🛑Preventing complications
🗣️Communication skills
😌Relaxation techniques
👣Diabetic footcare

This course is delivered by 2 tutors who have experience of living with Type 2 Diabetes.

‼️ IMPORTANT PATIENT NOTICE ‼️

The practice will be switching to a new clinical system on the 02/09/25. As a result, we will not be able to access ANY aspect of your medical records from 6pm on Wednesday 27th August.

🔴This means that we will be running at a significantly reduced service and will have the capacity to deal with urgent matters only.

❌Our online functions such as e-consult and the NHS Wales App will also be unavailable during this period.

💊REPEAT MEDICATION = If your repeat medication is due between Monday 25th August (Bank Holiday Monday) and Wednesday 3rd September, we ask that you put your request in before Tuesday 26th August. If you have enough medication to last you, please order after 3rd September.

❌Please note we will not be able to access your notes and issue routine repeat medication during this downtime.

🌈This transition will cause extra upheaval for all staff and patients, please be patient with us during this time. We apologise for any inconvenience this may cause.

Please contact the surgery if you are taken ill during this downtime in the normal manner. Alternatively, please use your local Pharmacy or ring 111 for advice.
